We are looking for a PRN Occupational Therapist to join our team in a Skilled Nursing & Long Term Care setting in Hobart! Occupational therapists treat disabled, ill, or injured patients with special equipment and the therapeutic use of daily activities. They help patients improve, regain, and develop the skills needed for day-to-day life and work. They may provide long-term patient care and acute patient care
Job Description:
· The Occupational Therapist (OTR)is responsible for the evaluation and treatment of residents who demonstrate disorders or neurological deficits, weakness, and paralysis. Evaluation and treatment will only be conducted after a written order is received from the physician.
· Is responsible for the screening of residents of the facility as follows:
o New facility residents: Within 24 hours of admission
o Long-term residents: Upon receipt of a “Significant change form”
o And /or upon referral from nursing
· The OTR is responsible for the completion of all necessary documentation, which includes but is not limited to the following items
o Initial evaluation forms
o Weekly progress notes
o Monthly Recertification Forms
o Discharge Summary
o All other required forms as designated by the facility and Regional Manager
· The OTR is responsible for attending all required facility meetings and in-services, including Medicare meetings, patient staffing, family meetings, and care plans.
· The OTR will in cooperation with the Program Manager and the Regional Manager, develop programs appropriate to the needs of the residents in the facility, to assist them in achieving their highest feasible level of functioning.
· The OTR will assist the facility in meeting the standards necessary to obtain state, federal, and other appropriate certification.
· The OTR will employ and follow all appropriate facility infection control policies and procedures and will respect and follow OBRA Resident’s Rights Guidelines
· The OTR is responsible for the appropriate scheduling of residents on caseload in addition to all required in-services and meetings
· The OTR will always present her/himself in a professional manner including professional conduct, professional communication, and appropriate dress while in the facility.
· Other duties as assigned by the Regional Manager and facility
Supervision:
The occupational Therapist will report directly to the Program Manager who is responsible for overseeing direct clinical skills, documentation skills, and compliance to meet all professional standards.
