Job Summary:

We are seeking a skilled and motivated Physical Therapist to join our team and make a positive impact on the lives of our patients. The Physical Therapist will be responsible for assessing, planning, and implementing individualized therapy programs for residents in a skilled nursing facility. The role involves working closely with a multidisciplinary team to deliver high-quality rehabilitative care that promotes functional independence and overall well-being.

Responsibilities:
- Patient Assessment: Evaluate patients’ physical conditions, mobility, and functional abilities to develop comprehensive treatment plans tailored to their needs.
- Treatment Planning: Create and implement personalized therapy programs that address specific goals, improve mobility, alleviate pain, and enhance overall physical function.
- Therapeutic Interventions: Administer therapeutic exercises, manual therapy techniques, and modalities to help patients achieve their rehabilitation goals.
- Progress Monitoring: Regularly assess and document patient progress, adjusting treatment plans as necessary to optimize outcomes.
- Patient Education: Educate patients and their families on exercise techniques, safety measures, and home care strategies to support ongoing recovery and maintenance.
- Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and address any issues that may impact patient progress.
- Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with facility policies and regulatory requirements.
- Compliance: Adhere to all safety and infection control protocols, as well as facility policies and procedures, to ensure a safe and effective therapeutic environment.
Qualifications:
- Education: Doctoral or Master’s degree in Physical Therapy from an accredited institution.
- Licensure: Current state licensure as a Physical Therapist.
- Experience: Previous experience in a skilled nursing or long-term care setting preferred, but not required.
- Personal Attributes: Compassionate, patient-focused, and dedicated to providing high-quality care. Strong organizational skills and attention to detail are essential.
