VNA of Ohio is now hiring a Licensed Social Workers (or LISW) for our Behavioral Health Group and your qualifications and skill set may be an ideal match for this opportunity. This is a PRN position covering Cuyahoga, Summit, Lake and Lorain Counties.

Responsibilities:
Job Summary:
Provides consultation in Home Health relative to psycho-social, financial and related issues. Coordinates necessary services involving other community agencies and services. May provide direct casework and counseling services. |
Essential Functions:
Illustrative duties may include, but are not restricted to, one or more of the following:
- Performs (or evaluates) initial assessment to determine social service needs of patient on physician referral.
- Develops a plan of care that directly stems from a comprehensive social work assessment with goals and appropriate social work interventions in conjunction with physician referral/orders.
- Provides (or assists in providing) direct social service interventions, including coordinating care plan interventions with other community agencies.
- Participates in the interdisciplinary team for the development and revisions of the Plan of Care.
- Re-evaluates and revises Plan of Care, as needed, with physician approval.
- Completes required documentation, including documenting services provided in the Clinical Record in accordance with agency policies and procedures.
- Demonstrate Social Work Clinical Competencies.
- Participates in (or provides) in-service programs and other programs and meetings as needed.
- Participates in interdisciplinary care conferences as needed.
- Provide social service interventions such as:
- Offering specific facilitative functions, including but not limited to advocacy for entitlements, mobilization of resources and coordination of medical/community services, assistance with alternative living arrangements.
- Identify “at risk” situations with appropriate social work interventions and needed community referrals.
- Facilitate planning for patients when level of care changes (i.e., alternative housing options and community referrals).
- Provide time limited supportive counseling as outlined in the individualized treatment plan and care givers.
- Maintain professional competency by participating in professional continuing education; staying informed on new developments and treatment modalities for clinical social work services related to medical/psychiatric patients.
- Performs other duties as assigned.

Qualifications:
- Master's degree in Social Worker required
- 2 years of post Master's experience in a health care setting (preferred)
- Demonstrated competence in medical social work, including the ability to work effectively in a multidisciplinary team approach.
- Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requiring significant collaboration.
- Strong planning and organizational skills.
- Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.
- Ability to work independently with minimal supervision.
- Demonstrated ability to maintain strict confidentiality.
- Demonstrated effectiveness in written and oral communication.

Required Licenses, Registrations and/or Certifications:
- LSW, LISW preferred
- CPR certification through the American Heart Association is required.
- Valid driver's license
