Being a Program Coordinator- Blended Case Management
You will be offered:
- Flexible Schedule (this is primarily a 9-5 position, however, there will be occasional need for evening flexibility)

- Pay rate of $44,000-$50,160 (dependent upon experience)

As a Program Coordinator you will:
- Be responsible for the delivery, coordination, supervision, and management of the Blended Case Management Department
- Manage compliance activities, weekly supervision, accountability measures, and operational standards within the department
- Assist with crisis intervention and serves as additional on-call support.
- Display leadership in attending meetings with community stakeholders, system partners, and BHMCOs while coordinating program activities
- Manage daily administrative functions of the case management
- Perform competency assessments as required and participate in program expansion projects.
The home office is located in Malvern, PA; this position requires daily presence at the Malvern office with some work-related travel to case management services within the local community, primarily in Chester County.
Our Blended Case Management Services help children, adolescents and young adults, ages 2 to 21, and their families navigate the world of social services. Our team works collaboratively with family members, funders and service providers to advocate on behalf of the child, ensuring care is coordinated, effective and responsive to the needs of each individual. Resources could include: behavioral health services, education, housing, employment, healthcare, legal aid and social skills. Through this program, individuals are required to meet with a case manager two times per month to ensure the effectiveness of the program. A 24-hour on-call service is available for individuals and their families for emergency crisis situations.
Devereux Pennsylvania Children’s Services (DPACS) serves children, adolescents and young adults with autism spectrum disorders, intellectual and developmental disabilities, behavioral, emotional and psychiatric challenges and disorders. Located in West Chester and Malvern Pennsylvania, services provided include psychiatric residential treatment (PRTF), acute inpatient psychiatric hospital (AIP), assessment services, community-based mental health services, community-based employment training and social skills training, and approved private school educational services to male and female children and adolescents (6-21 years of age) with diagnoses of intellectual disabilities, autism spectrum disorders, concomitant psychiatric and behavioral challenges often associated with experiencing some level of trauma in their lives.

Qualifications:EDUCATION: Master’s Degree in Human services or related field with 3 years of direct care experience, or BA degree with 5 years of direct care experience in which 2 years are as supervisor.
EXPERIENCE: 3-5 of experience in behavioral health care, mental health, or drug/alcohol program. Valid driver's license must be maintained.
