- The Home Friend Program Coordinator supervises the overall planning, operations, and financial aspects of the program. The Coordinator is responsible for the program functions including application intake and assessment, case management, record keeping, scheduling, training, and supervision of the Home Friend staff.
- The Program Coordinator is responsible to ensure that the Home Friend clients receive an appropriate referral, intake, and assessment and the Home Friends provide the best in home services to the clients.
- Ensure New Community is fulling all provisions, services and requirements of the County of Essex, Department of Citizen Services, Division of Senior Services Home Friends Program Procedure manual.
- Prepare and submit fiscal and programmatic reports to Essex County Division of Senior Services and NCC management.
- On an ongoing basis the Program Coordinator networks and perform outreach via distribution of flyers, door knocking, and presentations to solicit new clients within the Central Ward.
- Complete client referral and intake. During the process assess the clients needs by collecting geographical, and biographical information that will assist in determining if the client is eligible to participate in the Home Friend program.
- Collect proof of income to determine income eligibility
- Ensure that Home Friend workers received twelve hours of in-service training centered around the aging and disabled population.
- Perform regular site visit to evaluate Home Friend staff in their working environment.
- Maintain client files, case notes, service delivery records (flow sheet), supervisory contacts. Proof of income, grievance procedures

Required Experience:
Bachelor Degree in Social Work, Psychology, Nursing or the Human Services field:
- 2-5 years supervision experience.
- 2-5 years case management oversight experience
- Demonstrate the ability to document assessments and evaluations.
- Demonstrate patience, tact, enthusiasm, and a positive attitude toward the elderly and disabled.
- Ability to communicate effectively in English, both orally and in writing, bi-lingual a plus.
- Must be a licensed driver and have a personal vehicle that can be utilized for field visits
