Program Coordinator - Community & School Health Initiative

Northridge Hospital Medical Center is seeking a passionate and dedicated Program Coordinator to join our Community Health Work & Sustainable Outreach and Navigation (CHW SON) team. This position is grant-funded and will play a vital role in the daily supervison of Community Patient Navigators (CPN's), relays updates and orversees the implemtaion and refinement of the CHW SON project.
Responsibilities:- Develop and implement program activities, including community health education workshops, outreach events, and resource referrals.
- Manage program budget, track expenses, and ensure compliance with grant requirements.
- Develop and maintain program materials, including curriculum, presentations, and promotional materials.
- Coordinate with community partners and stakeholders to ensure program effectiveness and reach.
- Identify and engage with community members, particularly those from underserved populations.
- Build relationships with community leaders, organizations, and institutions.
- Conduct needs assessments and community health surveys to inform program development.
- Train, and supervise Community Patient Navigators (CPN's) to deliver program services.
- Provide ongoing support and mentorship to (CPNs), ensuring their skills and knowledge are up-to-date.
- Collect and analyze program data to track progress, measure impact, and inform program improvement.
- Prepare regular reports for funders and stakeholders, highlighting program achievements and challenges.
- Other duties as assigned
- BS/BA degree required in Health Administration Public Health or Public Policy degree preferred.
- Master's degree preferred.
- 2 years of experience as a Community Health Worker or System Navigator or 5 years’ of experience as in community based programs or social services.
- Minimum 1-2 years’ experience implementing community-based programs developing outreach strategies collaborating with community-based organizations and report writing.
- Basic knowledge of budgeting or accounting.
- Knowledge of local community agencies and resources.
- Ability to organize projects and establish priorities.
- Creative and analytical skills.
- Ability to travel locally for outreach events (some evenings and weekends).
- Grant writing experience preferred.
- Presentation and public speaking skills.
- Working knowledge of Microsoft 365 and Google Workspace products Zoom and Canva.
- Bilingual (Spanish/English) preferred.
- Fire Card (must obtain within 90 days of hire date. We offer the classes in the hospital)
