**UW Medical Center-Montlake - Post-Lung Transplant** has an outstanding opportunity to hire a **PROGRAM COORDINATOR / UW MEDICINE PATIENT SCHEDULER** .
**WORK SCHEDULE:**
+ Full-Time, Day Shift
+ Mondays - Fridays, 8:00 am - 5:00 pm **DEPARTMENT DESCRIPTION:**
As the only lung transplant program in the Northwest, our team performs up to 60 transplants per year and has transplanted nearly 1000 patients. We use a multidisciplinary team approach through all phases of the transplant process, with involvement from patients and families.
**POSITION HIGHLIGHTS:**
+ This position will coordinate and schedule extensive diagnostic tests and consultative appointments necessary for transplant evaluation, including independently prioritizing the needs of program clients, and coordinate pre transplant ambulatory (outpatient) continuum of care for lung transplant patients
+ Be a part of a team dedicated to supporting UW Medicine where Patients Are First **PRIMARY JOB RESPONSIBILITIES:**
+ Interact with providers, nurses and other team members to schedule all diagnostic tests as ordered by providers
+ Report patient and/or family/caregiver concerns regarding medical, social, financial, psychological or cultural needs to appropriate transplant team member in order to facilitate the planning, personal comfort, or safety for the planned test or procedure
+ Prepare schedule for diagnostic tests and clinic; interact with patient and family/caregiver to provide necessary program instructions and to discuss itinerary
+ Facilitate pre transplant ambulatory continuum of care steps including initial lung transplant appointment, transplant evaluation and waiting list phases of care including: test, procedure and appointment scheduling, outgoing referrals and coordination of care with PCP/pulmonologists' offices, and completion of missing outside provider documentation
+ Coordinate the receipt and distribution to appropriate team member(s) of required items such as: patient forms, medical records, lab and diagnostic results from UWMC, primary care physicians' offices, pulmonologists' offices and other medical facilities.
+ Maintain transplant charts; send records for scanning into EMR; file reports and clinical notes; file correspondence and regulatory documents **REQUIREMENTS:**
