As a Program Development Specialist - Social Worker, you will play a crucial role in our value-based care clinically integrated healthcare network. Your primary responsibility will be to develop, implement, and oversee Social Worker programs aimed at improving patient outcomes, enhancing patient experience, and optimizing the delivery of care within our network. You will collaborate with interdisciplinary teams to identify areas for improvement, develop strategies to address them, and evaluate the effectiveness of interventions specific to the scope of Social Worker. Your expertise in social work will be instrumental in addressing the social determinants of health and promoting holistic patient wellness. |

Responsibilities:Design and implement innovative programs to address the diverse needs of our patient population, with a focus on promoting value-based care and enhancing patient outcomes. Provide education and training to staff members on best practices in social work interventions, patient engagement strategies, and value-based care principles. Conduct thorough needs assessments to identify gaps in services, barriers to care, and opportunities for improvement within our network. Closely collaborate with interdisciplinary roles like the RN and CP program development specialist to develop programs. Utilize your expertise in social work to address the social determinants of health, such as housing instability, food insecurity, and access to transportation, that impact patient health and well-being. Lead efforts to monitor, evaluate, and continuously improve the effectiveness of existing programs through data analysis, performance metrics tracking, and feedback mechanisms. ?Develop and maintain quality assurance programs for Social Worker role to ensure compliance with regulatory standards. ?Monitor clinical outcomes to identify areas for improvement and implement necessary changes. ?Foster a culture of continuous quality improvement within the nursing and clinical teams. ?Develop a system for rigorous, repetitive, tiered clinical reviews for regional and market-based teams. Monitor and evaluate the effectiveness of programs and interventions, using data-driven metrics to assess outcomes and make evidence-based recommendations for improvement. ?Implement best practices in care delivery and continuously assess and improve workflows. ?Support initiatives to optimize resource allocation and operational effectiveness. ?Support Health System and partner affiliates clinical outcomes teams which defines and validates its evidence of clinical impact. ?Collaborate with Product and Clinical Operations teams to drive accuracy, precision, reliability, and efficiency in clinical workflow. Coordinate project timelines, milestones, and deliverables to ensure successful execution of program initiatives within budget and scope. Stay abreast of relevant healthcare regulations, guidelines, and quality measures to ensure program compliance and mitigate risks. Prepare and present regular reports on program outcomes, performance metrics, and key insights to leadership, stakeholders, and regulatory bodies as required. |
EDUCATION:
Required: Master's degree in social work (MSW) from an accredited program.
EXPERIENCE:
Required: 5 years of experience in social work, preferably in a healthcare setting.
Preferred: 3+ years of experience in Provider Credentialing preferred.
LICENSURE/CERTIFICATION/REGISTRY/LISTING:
Preferred: Licensed Clinical Social Worker (LCSW)
