Supervisory experience:
- A personal vehicle that is insured and meets state safety/licensure requirements;
- A valid driver’s license may require a chauffeur’s license;
- Must successfully pass criminal background and Employment Disqualification List checks prior to working with consumers;

- The main focus of this position is to coordinate the development and implementation of supported living services for individuals with developmental disabilities and employees, ensuring the delivery of quality services.
- This coordination occurs through the direct supervision of employees, support for consumers, and ongoing communication with family, community, funding, and licensure sources.
- All coordination must be conducted within the contract, state, and federal regulations, as well as Company Policies and Procedures.
- All candidates must be able to work in a fast pace environment, be detailed oriented, be a team player as well as work independently, and have great problem-solving skills.
Responsibilities:
Person-Centered Planning and Individual Support Planning:
- Actively participate in the person-centered planning process, including the development and implementation of Person Centered Plans (PCPs) and Individual Support Plans (ISPs)
- Design and implement support and teaching strategies targeted at assisting consumers with reaching their outcomes, as outlined in the consumer’s ISP
- Provide supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP, and teaching strategies
- Evaluate and revise support and teaching strategies as necessary to ensure progress toward the achievement of outcomes and action steps
Quality Assurance:
- Review all forms, notify the Case Manager of any incidents, and provide oversight for the follow-up plan with staff
- Maintain communication with family members, guardians, staff and Case Manager on a regular basis, particularly regarding policies, issues, and concerns
Supervision/Training:
- Coordinate in-service training that is needed to assist staff in fulfilling responsibilities related to consumer-directed support
- Provide direct support by providing services needed to protect the health and safety of the consumer, especially in a staffing crisis
- Communicate with the Interdisciplinary Team about concerns and findings
- Coordinate and assist staff with maintaining positive relationships
- Be available to respond to staff in case of an emergency
- Perform on-site evaluations for all field staff
