POSITION OVERVIEW:
The Program Manager will oversee the San Francisco AIDS Foundation's (SFAF) Health Access Points (HAP) portfolio, ensuring the delivery of high-quality services and the achievement of program goals. Health Access Points are "population specific one-stop shop or network of agencies/programs with a lead agency that provides an equity-focused, stigma-free, and low barrier access to person-centered, standard of care services regardless of HIV, Hepatitis C (HCV), or Sexually Transmitted Infections (STI) status" and are focused on eliminating health disparities in HIV, HCV, and STIs.
The HAP Program Manager will coordinate and manage services that include integrated testing, linkage to PrEP, PEP, HIV care, and overdose prevention, in addition to referrals and linkages to substance use and mental health services, and support for access to basic needs. The HAP Program Manager will SFAF leads the HAP reaching Gay Men and other MSM in collaboration with community partners, in addition to holding subcontracts with other community-based organizations as part of the Trans HAP, the Youth HAP, the Latine HAP, and provides capacity building and partnership with the HAP reaching Black/African American communities. The position will develop a network among the SFAF HAP program staff and facilitate linkages and connections between all SFAF programs and the HAP services.
The Program Manager will ensure that the HAP programs are reaching communities with effective comprehensive and coordinated services that the standards of care established for the HAPs. The Program Manager will provide technical assistance and capacity-building support to the HAP Network, with a focus on priority communities.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Oversee the implementation and management of HAP services, ensuring alignment with SFAF's strategic plan and organizational priorities.
- Collaborate with Finance & Development teams to steward resources efficiently and develop contracts and budgets that reflect the programs.
- Build and maintain partnerships with community agencies and stakeholders to enhance service delivery and program impact.
- Monitor program performance, ensuring compliance with funding requirements and contractual obligations.
- Lead a team of committed individuals, fostering a healthy work environment and promoting professional development opportunities.
- Engage in advocacy and policy work to advance the needs of communities most impacted by HIV, STI, and HCV.
- Partner with the San Francisco Department of Public Health and other HAP programs to ensure a seamless system of care.
OTHER DUTIES:
- Participate in required meetings and training.
- Ensure SFAF teams are updated on the HAP progress and network across the system of care.
ESSENTIAL QUALIFICATIONS:
- Strong understanding of public health, harm reduction, and the needs of communities impacted by HIV, STI, and HCV, and substance use issues, including overdose.
- Proven experience in program management, including overseeing client-facing programs and building effective internal policies and processes.
- Ability to lead and inspire a team, manage diverse projects effectively, and foster integration across programs.
- Excellent collaboration skills with both internal and external stakeholders.
- Commitment to SFAF's mission and values, with a focus on equity, health justice, and community-centered approaches.
EDUCATION AND EXPERIENCE:
- Bachelor's degree in public health, social work, or a related field.
- Minimum of 5 years of experience in program management or a related role within a non-profit or public health setting.
SUPERVISION:
- Supervision of the Sexual Health Capacity Building Coordinator

Skill in operating equipment such as a personal computer, copy machine and telephone system. Ability to perform routine bending, stooping, twisting, and reaching. WORK ENVIRONMENT:
This position is primarily located at 940 Street and 470 Castro Street, San Francisco, however it may occasionally require traveling to other office locations or off-site meetings as required by the job. Role may include evening and weekend coverage.
