River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups.
River Edge is actively searching for a Full-time Program Manager to join our Permanent Supportive Housing Program. This role involves overseeing and guiding team members and clients, coordinating housing and support services for homeless individuals, connecting them to necessary resources, conducting mental health assessments and counseling, developing individualized housing plans, and ensuring effective communication among service providers while monitoring client progress and offering crisis intervention when needed.

Duties & Responsibilities:
- Provide direction, oversight, and guidance to the team members and clients.
- Developing, securing, and coordinating services for the permanent supportive housing program.
- Connecting homeless individuals to housing supports/ resources as well as other resources identified as being needed, such as mental health, case management, skills building, and legal resources.
- Develop and maintain a network of community contacts. Facilitate referrals and linkages to community services and communication among client service providers.

- Utilize the coordinated entry assessment system.
- Monitoring and the permanent supportive housing program participant progress.
- Provide adjustment to housing changes and the desire to participate in other programs.
- Develop an individualized housing and service plan, including housing stabilization.
- Conduct the following mental health services; behavioral health assessments, initial visits and ongoing mental health counseling, substance abuse assessment, substance abuse and counseling and groups, discharge planning, including family contacts, Case management, and emergency crisis intervention.
- Demonstrates effective role modeling, delegation, and problem-solving and acts as a resource for the entire staff.
- Assures update of bio-psychosocial assessments with information relevant to treatment.
- Provides crisis intervention when appropriate. Use good clinical judgment to ensure the client's needs are addressed.
Qualifications:
- Fully Licensed in either - LAPC, LMSW, LAMFT, LMFT, LPC, LCSW)
- Master's degree in social service field (Social Work, Sociology, Psychology, Human Services) or related field.
- At least two (2) years of providing behavioral health service.
- At least two (2) years of community service board experience.
- At least four (4) years of relevant supervisory experience preferred.
- Valid Georgia Driver’s license.
- All applicants must pass a satisfactory background clearance and pre-employment drug test.
- Provide 2 Professional references (Former employers only, no family members).
Competencies:
Case Management & Client Assessment: Ability to conduct behavioral health assessments, housing stabilization plans, and mental health counseling, including crisis intervention and substance abuse support.
Resource Coordination & Referral: Skill in developing and maintaining a network of community contacts, facilitating referrals, and linking clients to essential housing and support services, including mental health, legal, and skills-building resources.
Program Monitoring & Evaluation: Proficiency in using the coordinated entry assessment system and tracking participant progress within the permanent supportive housing program.
Crisis Intervention & Problem Solving: Expertise in providing crisis intervention using sound clinical judgment, addressing clients’ urgent needs while guiding them through housing adjustments and participation in other programs.
Team Leadership & Collaboration: Competence in providing direction, oversight, and guidance to team members, effectively delegating tasks, role modeling, and fostering communication among service providers.
