MY CHOICE TRANSITION COORDINATOR / PROGRAM MONITOR
Serves at the OAAS My Choice Transition Coordinator (TC) for the My Choice LA (MCL) project. These positions are work from home positions and are domiciled to assure statewide coverage.
My Choice Louisiana (MCL) is a state program within the Louisiana Department of Health (LDH) and operated jointly by the Office of Aging and Adult Services and the Office of Behavioral Health. MCL grew out of an agreement between the state and the United States Department of Justice (DOJ.) Under the terms of the agreement, LDH must seek to transition and divert Medicaid recipients with Serious Mental Illness from unnecessary placement in nursing facilities. MCL staff work with individuals identified as meeting criteria for inclusion in the DOJ specified target population (diagnosis of serious mental illness, 18 years of age and older, and Medicaid eligible) and assist with desired transition into the community. The assistance provided by MCL staff includes but is not limited to assessment, identifying needed supports and services and securing such, and intensive transition management. The agreement requires LDH to have the necessary staff to carry out such tasks and the appropriate level of management and supervision to ensure tasks are delivered within a quality and consumer-driven framework. Thus, it is critical that OAAS implement the staffing necessary to support required activities.
OCCUPATIONAL SUMMARY:
The TC is responsible for the assessing, planning, coordinating, facilitating, and monitoring the transition of individuals from nursing facilities to community settings using guidelines established by OAAS. The TC directly reports to the Program Manager 1-B for their assigned area. The incumbent will coordinate, support, and track the activities of transition teams consisting of OAAS Regional Office staff, Medicaid enrolled support coordination agencies, Medicaid managed care organizations (MMCOs), and Medicaid providers in planning and executing transitions. The TC will provide technical assistance, facilitate communication between the team and identify and address specific issues preventing individuals from transitioning. The incumbent will participate in policy development and planning to maintain and/or enhance community capacity to support transition targets. This will include combining state and local initiatives and resources to support participants to transition. Incumbent may also participate in activities that involve multiple administrative units within the Louisiana Department of Health and across multiple state agencies.
Incumbent must adhere to pertinent laws, rules, regulations, policies, and procedures of LDH, OAAS, Civil Service and appropriate state/federal licensing and governing bodies. Performs other duties as assigned for smooth operation of the agency.

Minimum Qualifications
MINIMUM QUALIFICATIONS:
A baccalaureate degree plus two years of professional level experience in administrative services, economics, public health, public relations, social services, health services, or surveying and/or assessing health or social service programs or facilities for compliance with state and federal regulations.
SUBSTITUTIONS:
Six years of full-time work experience in any field may be substituted for the required baccalaureate degree.
Candidates without a baccalaureate degree may combine work experience and college credit to substitute for the baccalaureate degree as follows:
A maximum of 120 semester hours may be combined with experience to substitute for the baccalaureate degree.

ESSENTIAL DUTIES:
75% Plan, implement, and monitor institutional transitions. This includes facilitating and monitoring all transitions within their assigned regions or within the state. This may include but is not limited to:
+ Provides information regarding community transitions to nursing facility residents and their family or representatives.
+ Performs standardized transition assessments, and completes assessments required to determine level of care eligibility for OAAS HCBS services.
+ Prepare individual transition plans. Update transition plans as needed.
+ Obtain information from review of records, staff interviews, resident interviews, caregiver interviews, and personal observations in order to develop the transition plan.
+ Evaluates the resident's condition, preferences and needs, their support system and housing (if available) to develop transition plan.
+ Assists in locating housing and completing paperwork needed to apply for housing and/or rental subsidies.
+ Works with MMCOs and the Office of Behavioral Health to ensure appropriate behavioral health services are offered/provided as needed.
+ Returns signed documents for linkage to support coordination (SC) agencies and/or community-based providers. Assists in obtaining other documents as needed to accomplish transitions.
+ Tracks cases in order to identify issues that may slow down/prevent transitions.
+ Facilitates scheduled conference calls and meetings with transition team members.
+ Provides technical assistance and troubleshooting on difficult cases in effort to speed/facilitate transitions.
+ Conducts pre and post-transition visits with participant.
+ Evaluates the overall effectiveness of transitions and addressing issues that may arise once the person is living in the community.
+ Communicates with agencies/organizations providing support and training and other entities in order to maintain the individual’s community living.
+ Trains/educates nursing facility staff, as requested.
10% Develops and maintains relationships with local long term care Ombudsman and other professionals, such as property managers, who can be of assistance during transitions. This includes communicating with and providing, support and training for Office of State Ombudsman regional staff.
10% Develops and maintains an inventory of resources of services and products available in the community through vendors, local businesses, churches and other charity organizations. Develop relationships with these groups to assist in providing necessary services/products when requested, at the lowest price.
5% Identifies and communicates changes/revisions needed in current policies related to nursing transitions. Participate in activities involving other program offices/Medicaid, as requested. Other duties as assigned.
