Program Support Specialist - Trenton, NJ - Biogensys

We are hiring a Program Support Specialist for one of our clients in Trenton, NJ.

Job Description:

• Under the direction of a supervisory official, designs, plans, coordinates, implements new, and upgrades existing programs regarding opioid response.
• Specifically, this candidate will be responsible for conducting outreach efforts, onboarding new partners, and maintaining existing partner relationships through continuous engagement as well as conducting initial and follow-up training on Department resource platforms.
• This candidate will coordinate with a variety of referring organizations within their region to link individuals being discharged from these settings to services in the community through Department-approved platforms.
• Duties will also include coordinating with receiving treatment organizations to ensure a successful connection and engagement in care; Conduct patient follow-up/check-ins at 24 hours, as well as at 30, 60, and 90 days after the initial contact to confirm that they have successfully connected with the treatment organization; during which candidate will identify barriers preventing individuals from accessing or receiving treatment; and Identify areas for improvement in the overall process.
• They will act as a subject matter expert and provide solutions to complex issues involving established and new programs expanding awareness and education to the community on available resources during community events and conferences.
