Provider - Nurse Practitioner - Health Ministries Clinic

Our Family Medicine APRN's deliver direct patient care as part of the medical team in an integrated primary care/behavioral health clinical setting. Our providers work alongside behavioral health and nursing team members to manage patients' health and motivate lifestyle changes to optimize health while basing patient care on evidence-based practices within clinic protocols.

Key duties and responsibilities:

• Perform initial patient screenings, patient history, and physical examinations
• Update patient records and check records for accuracy at each patient appointment
• Analyze test data and determine the need for follow-up appointments and further treatment options
• Deliver quality primary care, including ordering lab tests, prescription drugs, and making referrals
• Serve as the resource patients need for ongoing care information and counseling
• Provide guidance for patients with critical conditions
• Participates in HMC's QA/QI and risk management activities

We are specifically looking for APRN's with previous clinical experience. Each HMC provider is supported by high-quality nursing staff, allowing them to focus their attention and skills on helping our community. Located in Newton, KS, Health Ministries Clinic is a high-functioning Community Health Center (FQHC) that seeks to promote and improve the quality of life by providing integrated healthcare services accessible to everyone in the communities we serve. We have a robust payor mix that reflects our community as a whole and maintain a commitment to serve all patients regardless of their ability to pay. We operate three main clinical service lines: Medical, Behavioral Health, and Dental. We also offer an extensive array of in-house ancillary clinical services such as Pharmacy, Lab, and Diagnostic Imaging (CT, X-ray, Dexa) not typically found in most outpatient clinic settings. Additional assistive services include transportation, on-site Spanish interpreting, case managers, care coordinators, diabetic education, and patient ambassadors.
