Purpose/Position Summary:
The primary purpose of this position is to provide group psychosocial rehabilitation services to adults within a long term care/nursing home setting. This position participates with the interdisciplinary team to develop, implement and evaluate effective therapeutic services via facility programming.

1. Direct Service
a. Provide socialization/stimulation to residents in the facility.
b. Monitor or assist with facility smoking programs.
c. Conduct socialization/psychosocial groups as directed by supervisor.
d. Assist CNA/Nursing staff at meal time with passing trays and clean up.
e. Assist with crisis intervention as needed.
f. Assist residents going into the community who require supervised access.
g. Assist residents who may require 1:1 sessions and/or other interventions.
h. Provide 1:1 sessions under the direction of the Social Service Director/PRSD/PRSC to promote self-maintenance, socialization, group attendance, etc.
i. Assist residents when going to appointments outside the facility.
j. Assist with all ancillary appointments in the facility: dentist, podiatrist, audiologist and psychologist.
k. Assist residents with the management of personal items.
l. Assist with contraband management, including room searches.
2. Record Keeping
a. Planning Groups/Individual Sessions.
b. Maintain accurate records, including documentation of 1:1 sessions at a minimum of one time per month or more frequently as indicated and group session attendance/summaries.
c. Conduct groups/activities as components of the facility’s psychiatric rehabilitation program around: self-maintenance, social functioning, community living, occupational skills, substance abuse interventions and symptom management.
d. Assist with setting up for group activities and ancillary service programs, as well as encourage resident participation.
e. Participate in treatment, program planning and in-service trainings.
3. Facility Training and Activities:
a. Participate in treatment, program planning and in-service training.
4. Miscellaneous:
a. Ensure that all elements of the facility’s Abuse and Neglect policies are maintained and enforced.
b. Ensures HIPAA compliance is maintained at all times.
c. Possess ability to make independent decisions when circumstances warrant such action.
5. All other duties as assigned.
Required/Preferred Education & Experience:
1. High School diploma or GED equivalent required. CNA certification preferred.
2. Minimum of 1 year experience in a skilled/long term care facility or a mental health facility preferred.
3. Bachelor Degree in Psychology or Social Service
Physical Demands:
The physical demands of this position must be met by an employee to successfully perform the essential functions and duties of this position, with or without accommodation.
This position is very active and requires prolonged walking, standing, sitting, bending, kneeling and crouching constantly during shift.
The employee must frequently move (lift, push, pull, etc.) objects and residents weighing over 50 pounds.
The employee must be capable of effectively communicating with residents, family members, peers, visitors, government agencies/personnel and the general public.
The vision abilities required by this position include: close/distance, color, peripheral, depth perception and ability to adjust focus.
