Role Responsibilities:

1. Conduct comprehensive psychiatric assessments for elderly nursing home residents.

2. Develop tailored treatment plans based on patients' mental health conditions and diagnoses.

3. Administer medication management and make necessary treatment plan adjustments.

4. Collaborate with multidisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care.

5. Provide counseling and psychotherapy services to patients and their families.

6. Educate patients and their families about mental health conditions, treatment options, and self-care strategies.

7. Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.

8. Actively engage in clinical meetings, case conferences, and quality improvement initiatives.

Requirements:

1. Hold a valid Nurse Practitioner license in New York, specializing in Psychiatry (PMHNP).

2. Demonstrate the ability to work autonomously and efficiently manage a patient caseload.

3. Possess strong diagnostic skills and extensive knowledge of psychiatric medications and treatment modalities.

4. Exhibit excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.

5. Be proficient in electronic health record (EHR) systems and possess basic computer skills.
