Our client, a multidisciplinary community health center in Spring Valley, NY, seeks a Psychiatric Nurse Practitioner to join our Behavioral Health team
Type: Perm
Schedule: Full Time Weekdays Monday – Friday either 8am-4pm or 9am – other schedule are available - Additionally 1 Saturday per month
Location: Spring Valley, NY
Summary:
The Psychiatric NP will provide behavioral health treatment for a wide-variety psychiatric disorders and help with all aspects of patient care including diagnosis, treatment, collaboration and consultations.
Job description:
* Assesses the patient's general mental health condition by collecting medical histories, administering comprehensive psychiatric evaluations and identifying issues that may be affecting the patient's state of mental health.
* Interprets and integrates data to determine preliminary diagnosis and creates an individualized therapeutic plan for mental health problems and psychiatric disorders based on evidence-based standards of care and practice guidelines.
* Prescribes psychotropic drugs and related medications to treat symptoms of psychiatric disorders and improve functional health status based on clinical indications of a patient's status, including results of diagnostic and lab tests.
* Identifies, measures, and monitors clinical and related behavioral outcomes to determine the effectiveness and appropriateness of the plan of care.
* Educates patients and /or family as to the nature of the psychiatric condition and provides appropriate instructions on proper intervention and treatment.
* Collaborates with the psychiatrists and other staff members in the team in managing acute and long-term medical and psychiatric/mental health needs of patients.
* Documents medical history and treatment plans effectively, concisely and accurately. Maintains patient confidentiality.
* Coordinates referrals and ongoing access to primary and other health care services for patients.
* Provides consultation to and seeks consultation from other health care clinicians to enhance quality and continuity of care.
* Collaborates with other departments in the health center to improve and enhance communication and quality of care.
* Provides coverage for other providers after hours or when away (e.g. vacations or time off).
* Complies with applicable policies and procedures and supports the Compliance Program.
* Contributes to organizational operations by performing other duties as may be required .
Qualifications:
* Active NY DEA Registration
* BSN, MSN preferred, successful completion of an approved nurse practitioner psychiatric program
* Current registered nurse license and certification in New York State
* Eligible to Work in the US
