JOB DEFINITION:
The Psychiatric Rehabilitation Care Coordinator (PRCC) functions as a member of a multidisciplinary treatment team to provide skill-building, vocational, and care-coordination services to individuals diagnosed with mental health and/ or substance use disorders. The PRCC partners with clients to identify social, community, and skills-based needs, and works with individuals to develop skills needed for meeting those needs. This role directly serves clients, engages with clinical treatment team members, and advocates with community-service agencies to reduce reliance on the behavioral health system, increase wellness self-management, and promote recovery.
EDUCATIONAL REQUIREMENTS:
Bachelor's Degree in one of the following fields: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy. Degrees in other related areas may be considered. Master's Degree with current state licensure preferred.
EXPERIENCE REQUIRED:
At least one (1) year of human service experience required. Experience should be in advocating for and providing direct services to underserved populations, including individuals with severe mental health or substance abuse disorders, developmental disorders, and/or physical disabilities. Experience with community-based services, including vocational supports and linkage to available benefits & resources essential to independent living is preferred.
DUTIES AND RESPONSIBILITIES:
- Assists in the evaluation process and in identifying a problem list, short-term goals, and long-term goals to be incorporated in clients' treatment plans.
- Participates in the development/documentation/review and update of client centered treatment plan in consultation with clinical team members to ensure focus on desired goals.
- Provide psychiatric rehabilitation and case management services in individual and/ or group formats to individuals and supportive family members in various locations, including in-office, in-community, in-home, and via telehealth, based on client needs.
- Provides telephonic as well as face-to-face outreach, engagement and service planning in the field.
- Helps client develop or regain physical or mental functioning or adjust to disabilities by implementing programs involving prevocational, vocational, education, independent living skills, activities of daily living, educational, recreational, and social activities.
- Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.
- Promotes maximum independence by selecting and constructing therapies according to individual's physical capacity, intelligence level, and interest.
- Maintains patient records, statistics, and other related reports that reflect patient treatment in an organized, timely and accurate manner (i.e. evaluation, daily treatment record, physician update, discharge summaries, collateral contacts) within the scope of staff education and licensure.
- Acts as a resource to team members in the areas of rehabilitation, care coordination, and linkage to community resources.
- Attends and participates in multidisciplinary team meetings and internal/ external trainings to provide input/feedback, review client status, update plans and goals, and review outcomes.
- Participate in ongoing supervision, training/education and internal/external meetings as needed to ensure a high quality of service delivery and continued professional growth.
- Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
- Complete QI Training during onboarding and participate in QI refresher training as needed.
