About the Job
Nonprofit Human Services Organization is seeking the following:
Psychiatric Rehabilitation Specialist/ Case Manager (Full Time)
To work with adult and youth clients diagnosed with a mental illness in Dorchester County, and adult clients in Talbot County in Maryland, providing mental health support services. Strong communication, written, oral and team work skills required.
Full-Time Hours for Youth Program: Monday - Friday - 9:30a-5:30p with the flexibility to work 10-6, if needed.
Full-Time Hours for Adult Program: Monday - Friday - 8a-4p, 8:30a-4:30p, or 9a-5p, depending on program needs.
Essential job functions and requirements include:
- Team oriented, flexible, energetic, and has a commitment to working with youth and adult clients experiencing a mental illness

- Delivering services to include individual meetings and leading groups both on property and in the community

- Completion of required documentation including service notes, assessments, treatment plans, etc. through an electronic health record

- Linking clients to resources in the community
```{=html}
```
- Work collaboratively and professionally within a team setting, with other agencies, and client support network
```{=html}
```
- Provides transportation in company vehicles to clients for service delivery
```{=html}
```
- Participates in on-call rotation
```{=html}
```
- Maintains valid driver's license, a clear driving record, and has evidence of auto liability insurance

```
- Background check required and maintains a clear criminal background profile, willing to undergo and pass drug testing

```
- Applicant must be at least 21 years of age

```
