Psychotropic Nurse - Aperion Care Chicago Heights

SUMMARY: The RN Nursing Supervisor performs professional supervisory nursing practice in the care of the physically and mentally ill. Work involves responsibility for supervising the provision of quality nursing care given at the facility. The position is responsible for maintaining the quality and adequacy of nursing practice; for coordinating nursing programs; for recognizing and assisting in solving professional, administrative and supervisory problems. Position exercises considerable judgment in applying professional knowledge in solving nursing problems within established policies and practices.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Coordinates the care of patients on assigned shift. Communicates shifts concerns by unit to appropriate staff (via e-mails, voice mail, etc.) .
• Review of 24 Hour Report and review patients as indicated for change of condition
• Serves as administrative authority for the smooth function of all units. Notifies Director or Nursing, Assistant Director of Nursing, Scheduler/Central Supply of urgent concerns during the shift. When the concern is non-urgent, notifies manager by phone message or e-mail.
• Receives employee call-offs. Records call-offs and updates schedule. Notifies managers/supervisors of call-offs in their units via voice mail. Replaces for absences and vacancies for upcoming shift(s). Assists staff to pick up hours (call-in phone, e-mail, vacancy lists, 72-hour availability list).
• Works with the Staffing Coordinator and Director of Nursing to assure that vacancies are filled in advance.
• Performs rounds to monitor the conditions and care of acutely ill patients
• Supervises staff and assures survey compliance on a daily basis. Provides guidance and support to staff to better plan work flow.
• Responds to code blue, medical assist situations. Assists staff, as needed.
• Provides clinical expertise in medical management, crisis and behavior management techniques.
• Audits charts routinely. Provides clinical expertise in documentation, including MOS, Medicare PPS, acute illness charting, weekly summaries and care planning.
• Meets routinely with the Director of Nursing and Assistant Director of Nursing.
• Assists with the development and implementation of nursing policies and procedures .
• Attends facility Quality Assurance and Performance Improvement Meetings.
• Be responsible for getting all psychotropic consents for all new residents and/or any new medication orders. Must have 2 nurses signature for ALL consents
• Be responsible to ensure Psychiatric Evaluations are completed within 14 days upon admission and every year, thereafter and as needed (with significant changes)
• Be responsible for making sure all residents are seen by psychiatrist per schedule.
• Ensure psychiatric progress notes are completed monthly and PRN
• Be responsible that psychotropic medication reduction be done Q6 months, and monitor effectiveness. If not favorable, psychiatrist must be altered and addressed. Intervention?
• Be responsible the AIMS assessment is completed upon Admission and Q6 months.
• Be responsible for administering DECONATE injections as scheduled and recording.
• Ensure behavioral documentation is complete for all behavioral incidents.
• Direct the day-to day functions of the nursing assistants. Meet with your assigned nursing staff, as well as support personnel, in planning the shift's services, programs and activities . Make written & oral reports/recommendations concerning the activities of the shift as required. Admit, transfer and discharge residents as required, Provide leadership to nursing personnel assigned to your unit/shift.
• Complete & file required recordkeeping forms/charts upon the resident's admission transfer and/or discharge. Receive & transcribe telephone orders from physicians & record on the Physician's Order Form. Chart nurse's notes in an informative & descriptive manner that reflects the care provided to the resident as well as the resident's response to the care. Fill out and complete accident/incident report and submit to Director as required. Performed routine charting duties as required and in accordance with established charting and documentation policies and procedures.
• Prepare and administer medications as ordered by the physicians
• Be responsible for petitioning residents out for psychiatric evaluations and treatment

1. Notify family, POA/Guardians and document for the record.
2. Making arrangements for transport with ambulance company and document for record
3. Alerting staff on resident's hospital transfer
4. Educating/ReEducating nurses on process with petitions, if not available to do it

• Be responsible for ongoing Quality Assurance with Psychotropic Program.
• Be responsible for provision of psychotropic program at monthly QA Meeting submitting to Administ rator . Provided completed audit tools (forms)
• Be responsible for provision of needed information during annual IDPH surveys
• Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the tacility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Create and Maintain an atmosphere of warmth, personnel interest and positive emphasis, as well as a calm environment throughout the unit and shi ft . Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with all psychiatric physicians and document visit in PCC Charting, adjust any new medication orders and document on PCC order summary; fax to pharmacy and initiate 72 hour follow up or psychotropic medication changes and document any ancillary orders (i.e. appointments/consultants/ etc)
• Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressing/bandages, packs, colostomy and drainage bags, taking blood, giving massage and range of motion exercises , etc.. as required
• Monitor your assigned personnel to ensure that they are following established safety regulations and for educating/reeducating nurses on assisting with behavioral documentation in the use of equipment and supplies.
• Be responsible to intervene with all resident behavior(s), documenting, administering PRN medication(s) as needed and assisting nurses as nee ded.
• Maintain confidentiality of all resident care information including protected health information. Report know or suspected incidents of unauthorized disclosure of such information. Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident's room
• Provide IV and IV si te care as needed.
• Performed other duties as assigned.
• Be responsible for performing ON CALL nursing management rotation

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.

• Registered Nurse with current unencumbered state licensure.
• Supervisory experience in a team leader role.
• Work experience as a professional nurse.
• CPR certification.
