Job Summary:
The Patient Care Coordinator is accountable and responsible for managing the clinical care and course of treatment of a number of patients. Coordination of care is accomplished through collaboration with physicians, associate staff, and other health care providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process. The Patient Care Coordinator is accountable to the Director of Nursing. Demonstrates the knowledge and skills necessary to provide care appropriate to the age, Cultural and Religious beliefs of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The Coordinator will function as an extension of the Director with oversight for the practice of nursing.
Performance Expectations:
ï‚· Meets established standards of practice of assessment and reassessment from admission to discharge as evidenced by appropriate documentation in the medical record
ï‚· Consistently plans patient care in conjunction with other disciplines to meet the goals and individualized needs of the patients
ï‚· Is competent in patient interventions as evidenced by safe administration of medications, treatments, procedures, infection control, and addressing changes in status to safely care for patients
ï‚· Consistently evaluates the practices of care delivered to patients and adjusts care according to established standards of practice and physician’s orders
ï‚· Consistently educates patients and family on equipment, medications, safety, pain control, and discharge needs
ï‚· Completes accurate and timely documentation
ï‚· Is competent to access and utilize the EMR for the provision of patient care
ï‚· Demonstrates the knowledge and skills to safely and appropriately fulfill job specific responsibilities related to safety, security, emergency preparedness, control of hazardous materials, life safety, and utility systems as outlined in the hospital’s environment of care plan
ï‚· Utilizes knowledge of age specific needs of patients, from pediatric to adult in performance of duties and responsibilities
ï‚· Able to safely utilize equipment including, but not limited to: Defibrillator, Automated Blood Pressure Machine, Infusion pumps, Fluid warmer, Rapid Infuser
ï‚· Is able to manage staff as well as the clinical care and course of treatment of multiple patients
ï‚· Responsible for making patient assignments and monitoring breaks and lunches
ï‚· Demonstrates the ability to care for the cardiac patient and identify lethal cardiac dysrhythmias
ï‚· Demonstrates the ability to assist with set up and maintenance of sterile field during procedures
ï‚· Demonstrates the ability to perform moderate sedation
ï‚· Keeps a clean, safe, quiet, and efficient work environment
ï‚· Demonstrates the ability to communicate effectively with staff, managers, Physicians and executive team
ï‚· Demonstrates critical thinking abilities
ï‚· Responsible for staff scheduling and assignment of duties.
ï‚· Demonstrates the ability to appropriately delegate tasks
ï‚· Responsible for assisting with the care of ER patients
ï‚· Responsibly supervises the delivery of care and duties of all departments in relationship to the overall safety of staff, patients, and visitors.
Qualifications:
Education/Skills
Graduate of a recognized non-online RN program.
Work Experience:
A minimum of two years as a staff RN in an acute setting is preferred; a Minimum of 1 year of ER experience is preferred.
Certification/Licensure-DUE UPON HIRE
ï‚· Licensed RN able to practice within the State of MS

ï‚· Basic Life Support Within 30 Days of Employment Required
ï‚· Advanced Cardiac Life Support Within 90 Days of Employment Required
ï‚· Basic Coronary Care Within 60 Days of Employment Required
ï‚· NIH Stroke Scale Within 1 Year of Employment Required
ï‚· Pediatric Advance Life Support Within 90 Days of Employment Required
ï‚· Moderate Sedation Within 90 Days of Employment Required
ï‚· Rhythm Strip Interpretation for Techs Within 60 Days of Employment Required
ï‚· Trauma Nursing Core Course Within 180 Days of Employment Preferred
