Schedule: 8:00 AM to 5:00 PM, Monday - Friday
Must be available to work additional hours as needed, including weekends, holidays, and on-call
Facility Information:
- Our facilities are Intermediate Care Facilities for individuals with disabilities (ICF/DD-H and ICF/DD-N)
- Caregiver (DSP) to people supported ratio is usually 1:3, sometimes as high as 1:1 support
- Nursing caseload typically ranges from 6-15 residents
- Our home sizes are small, including 6-bed and 15-bed facilities
- High-acuity care is limited or prohibited by facility regulations
- Our Immediate Care Facilities provide wound care per licensing requirements, ranging from simple wounds to a maximum of stage I pressure ulcers
- Hospice care is very limited and is provided collaboratively with professional hospice care providers who come to our homes
Our Client is the is a leading provider of care services in Intermediate Care Facilities (ICF) for individuals with disabilities. Our client is hiring a Qualified Intellectual Disabilities Professional (QIDP) to develop and monitor training goals for adults with disabilities at our group homes. As a QIDP, you are responsible for working closely with our people supported to understand and gauge their level of physical and cognitive ability for the purpose of designing training goals. The QIDP, in the absence of the Administrator, may be responsible for assuming duties and responsibilities of the Administrator, including being on-call for emergency situations. The QIDP works closely with the Administrator, Registered Nurse (RN), Director of Program Services, and other Momentum staff.
Case Manager - QIDP Essential Responsibilities:
- Promotes a friendly, comfortable, and safe environment
- Respects and promotes the right of individuals
- Designs training programs for people supported
- Supervises the development and implementation of each persons Individual Service Plan (ISP)
- Oversees and monitors individual goals, objectives, training programs, feeding programs, proper feeding techniques, and the use of adaptive equipment
- Acts as liaison between facility and community programs, bus company, and other agencies
- Documents monthly individual progress
- Initiates review as necessary for modifications or adjustments needed for ISPs
- Oversees evaluations conducted by consultants and ensure they are completed in a timely manner
- Supervises and implements Behavior Management Programs
- Oversees Direct Support Professional (DSP) evaluations
- Assures quality care is provided to all people supported
- Monitors and secures peoples supported financial accounts
- Follows and implements company policies
- Attends required in-service trainings
- Assists with personal care, hygiene and other daily living needs
- Performs other duties as assigned to meet department and/or business needs
Case Manager - QIDP Qualifications:
- Must be 21 years of age
- Bachelor’s degree in human services sociology, special education, rehabilitation counseling or psychology; required
- Must maintain valid and current proof of legal right to drive in California; required
- Current CPR Certificate
- Must be able to work on-call weekends and after hours
- Strong desire to work with individuals with developmental disabilities
- Ability to work in a fast-paced environment
- Strong written and verbal communication skills
- Ability to maintain confidentiality
- Ability to communicate and work professionally and cooperatively with others
- Ability to run, push, pull, bend, squat, kneel, lift up to 50 lbs., and transfer individuals
- Successfully pass all post offer, pre-employment screening requirements, including criminal background check, physical exam, drug screening, and provide proof of eligibility to work in the United States
