Quality Abstractor Coordinator - Henry Community Health

• Works under general supervision in carrying out the functions of chart review for quality assurance, accrediting bodies (i.e. ACHC, ISDH, etc.), infection control and risk management.
• Performs analysis of each case assigned, reviewing the patient care documentation utilizing pre-established screening criteria.
• Interacts with physicians and other health care workers to assure appropriate documentation is entered into the EHR.
• Identifies patient care variances and reports findings, which are disseminated to medical staff through committee and service meetings.
• Reviews medical records and abstracts a vast amount of clinical documentation to support the quality improvements and regulatory reporting requirements.
• Reports possible issues/concerns to the director of quality/safety in order to enhance performance improvement opportunities.
• Learn and understand the various terminologies as it relates to multiple databases. 
• Communicates any measure changes as appropriate.

• Current IN licensure as a Registered Nurse (BSN preferred)
• Previous experience in abstracting (preferred)
• 3-5 years clinical experience, which should include but not limited to medical-surgical nursing or related experience.
• Work requires a basic knowledge of pathophysiology, the nursing process, nursing interventions, pharmacology 
• High level of verbal and written communication skills 
• Knowledge of medical terminology, levels of care, treatment modalities and the current health care delivery system
• Computer proficiencies to enter and retrieve data, to assemble and organize data, to use, analyze and integrate data from diverse sources
• Must have an interest in continuous education opportunities
