Position Summary:
The Quality Care Coordinator works in collaboration and continuous partnership with “high-risk” patients, their family/caregiver(s), providers, and community resources in a team approach to improve health outcomes.
Essential Functions:
- Create and promote adherence to a care plan, developed in coordination with the patient, primary care provider, and family/caregiver(s).
- Monitor adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner, and facilitate changes, as needed.
- Assists as needed with the collection of Quality of Care monitoring
- Completes necessary documentation including assessment, progress notes, and care plans as applicable and in accordance with program and agency standards.
Job Responsibilities:
- Assess patient’s unmet health and social needs using standardized tools, and pre-visit planning, to facilitate patient access to appropriate medical and specialty providers.
- Increase continuity of care by managing relationships with care team providers, including, but not limited to, transitions of care, and referrals.
- Educate and connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.
- Serve as the contact point and informational resource for patients, care team, family/caregiver(s), payers, and community resources.
- Create ongoing processes for patient and family/caregiver(s) to determine and request the level of care coordination supports they need, and desire, at any point of care.
- Conduct patient engagement activities to promote timely access to appropriate care; including care associated with chronic conditions, such as Diabetes, increase use of preventive care services, reduce emergency room utilization, and hospital readmissions.
- Document all care plans, engagement efforts and successful contacts in Liberty’s designated electronic health record.
- Maintains effective professional relationships with other service providers and community organizations in the interest of clients’ and program goals in accordance with the Agency Policies and procedures.
- Performs other duties, responsibilities and related tasks as assigned.
Qualifications:
- Bachelor’s degree in related field or Associates and one (1) year experience or more.
- Experience with NextGen electronic health record, preferred.
- Knowledge of quality care measures, including HEDIS and CMS.
- Local knowledge about, and connections, to community services and health care providers
- Must have a valid New York State driver’s license and access to reliable transportation.

Pay Range: $23/hour to $25/hour.
