REACH Kidney Care is a relationship-driven company. The REACH Provider Partnerships Coordinator will play an important role in supporting, maintaining, and developing relationships with nephrology practices, health systems, hospice providers, and more.
The REACH Provider Partnerships Coordinator will work with the REACH operations team, the patient engagement team, the data analytics team, and the interdisciplinary team to ensure communication and collaboration on the operational aspects involving provider relations.
Schedule: Full-time, typical schedule is five 8-hour shifts, Monday - Friday

What You Can Expect:
- Support, maintain and develop relationships with nephrology practices, health systems, hospice providers, etc.
- Works under the direction of the REACH Provider Partnerships Manager to organize, innovate and improve the Provider Relationship operations.
- Work closely with the regional Care Manager Managers to develop strategies for engaging nephrology practices and closely communicate on operational items that are being implemented.
- Work closely with the patient engagement team to communicate items from the physician practices that impact the patient engagement team’s workflow.
- Work closely with the data analytics, clinical team, and REACH Provider Partnerships Manager to develop actionable reports for physician practices.
- Correspond with the nephrology practices on patient eligibility files, as well as operational and quality data reports.
- Send and receive e-mails to nephrology practices in a professional and in a clearly communicative manner regarding operational and quality items.
- Willingness to learn REACH’s operational details within each contract to be able to answer questions from a partnering physician practice.
- Attend team meetings and provide input when appropriate

- Bachelor’s Degree Preferred
- Strong working knowledge of Microsoft Office Suite
