Summary of duties:
The Recovery Coach at Outer Cape Health Services (OCHS), reporting to the Clinical Manager of Recovery Services, provides essential support to individuals in or seeking recovery from substance use disorders. This role involves meeting participants wherever they are in their recovery journey, using a "multiple pathways" approach to offer inclusive, personalized support.
Key responsibilities include providing individual and group peer support with a trauma-informed, person-centered approach; connecting participants to local resources and recovery support systems; and developing individualized wellness plans tailored to each participant's recovery goals.
The Recovery Coach also tracks and documents progress in the electronic medical record system, facilitates case conferences with partner agencies, and advocates for participant autonomy by equipping them with resources and skills to sustain their recovery. Additionally, the Recovery Coach participates in required training, meetings, and supervisory sessions to ensure the highest quality of care.
A Day in the Life of This RoleAs a Recovery Coach at OCHS, each day involves connecting with individuals in early recovery from substance use disorders. Guided by personal recovery experience, the Recovery Coach helps participants access resources, establish support systems, and develop personalized wellness plans. Meeting participants where they are in their recovery journey, Recovery Coaches often conduct sessions in community spaces like peer recovery centers or cafes to create a comfortable and accessible setting for support. The role includes ongoing collaboration with other support services and regular progress tracking to facilitate a comprehensive, individualized recovery journey.
Essential Functions
- Serve as a fully integrated team member, who identifies as a former recipient of recovery services, and now uses that experience to provide individual and group support to individuals receiving or seeking treatment.
- Connect participants to resources, supports, and opportunities in the larger community outside of OCHS's mental health system.
- Utilize fundamental principles of peer recovery through recovery-oriented, person-centered, voluntary, relationship-focused, and trauma-informed
- Develop and implement a flexible but specific work plan that supports the management and triage of multiple priorities and needs, while ensuring adherence to a treatment timeline.
- Connect with new participants to identify needs and develop a wellness plan.
- Assist in developing and sustaining natural support
- Provide peer support to engage participants in a recovery partnership based on the principles of mutuality.
- Provide services at times coordinated with participants and in publicly accessible locations such as peer recovery centers, restaurants, coffee shops, etc.
- Adhere to applicable laws, regulations and agency policies governing the disclosure of client identifying data and confidentiality.
- Attend all mandatory meetings, training, group supervisions, and individual supervisions.
- Track patient progress through contact and ongoing reassessment, collecting and inputting all required data into the required systems (e.g., electronic medical records).
- Facilitate and coordinate follow-up case conferences with referring agencies.
- Adhere to established case management standards of care.
- Coordinate and lead informal groups related to peer support, recovery, resiliency, and resources.
- Advocate for the people with whom they work, promoting their autonomy, protecting their rights, and providing access to resources, information, skills and support.

Work environment:
This position primarily requires sitting, standing, and computer use, along with daily travel to various locations. The Recovery Coach will conduct meetings in a range of settings, such as peer recovery centers, cafes, or other safe public spaces.
What we need from you:
- High school diploma or equivalent required, post-secondary education in social work or psychology preferred
- Experience in the profession of mental health, substance use, trauma & related social services
- Recovery Coach certification preferred
- Must have experience in addiction recovery services
- Valid driver's license and access to reliable transportation.
- CPR and First Aid certification
- Experience with EPIC or other electronic medical records system
- Case management, navigation, or community health worker experience
- Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
- Excellent written and verbal communication skills - bi-lingual helpful
- Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.
- Excellent judgment and ability to solve problems in a timely manner.
- Ability to adhere to strict confidentiality standards.
- Demonstrated ability to work effectively both as a team member and independently.
- Demonstrated ability to work in a culturally diverse and inclusive environment
