About the Job
Under the supervision of the Adult Community Clinical Services (ACCS) Assistant Director, the ACCS Recovery Specialist provides case management and clinical support for ACCS persons. Works with licensed clinicians to develop a comprehensive recovery based services to adults with serious mental illness. Participates as part of a multi-disciplinary team and coordinates/evaluates program services as required by client specific action plans. Requires valid driver’s license and access to personal vehicle for client transportation.
- Provides case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team
- Works with licensed clinician in developing, implementing, evaluating and revising overall treatment goals and plans in conjunction with the team
- Provides individual supportive therapy and symptom management, ensuring immediate changes are made in the treatment plans as clients’ needs change
- Educates and supports clients’ families, and advocate for clients’ rights and preferences
- Provides on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff
- Documents client progress to maintain a permanent record of client activity according to established methods and procedures
- Reviews progress notes monthly
- Provides treatment, rehabilitation, and support services, with some interventions directed or performed by staff with specialty training and skills (e.g., vocational specialists)
- Participates in providing substance use and treatment services as needed
- Assists with the development of individual behavioral support plans as needed
- Communicates with guardians, family members, doctors, hospitals, therapists, Human Rights Coordinator, and DMH regularly to coordinate schedules, supports and services
- Reviews implementation and progress of interventions
- Assists in client intake and assessment processes
- Explores social & support opportunities for individuals to participate in the community
Required Skills
- Demonstrated ability to work successfully at coordinating care across a continuum of care required
- Must demonstrate the following characteristics: Flexibility with schedule, ability to function as a team member in assessing the needs of individuals, the ability to contribute in the development of policies and procedures, creativity and resourcefulness in seeking pathways that will provide support to the program
- Bilingual skills and/or experience with multicultural populations preferred
Required Experience
- Master's Degree in Psychology, Social Work or related field
- Must have at least 2 years’ experience working with individuals with SPMI
- Familiarity with principles of psychiatric recovery required
