Jubilee operates 11 properties totaling 465 units of affordable housing, with another four properties and 120 units under development, and offers robust programming for residents and community members pursuing life goals.

Position Description
The Reentry Case Manager reports to the Reentry Program Director and works collaboratively in Jubilee's multidisciplinary approach to supporting returning citizens. The Case Manager works collectively amongst the team to create a transformative community experience, where peers support each other and residents receive support and accountability in their reintegration journey. The Case Manager will empower residents by working with them to assess their needs, develop Goal Plans, and monitor progress towards individual goals. The Case Manager will also lead life skills groups, meet with residents individually, and facilitate interdisciplinary approaches to care based on resident needs. The Case Manager will assist residents in securing and maintaining employment, establishing a budget and savings plan, and developing a long-term housing plan for successful transition at program completion. These activities will be executed through 1-on-1 engagements and group activities which will occur virtually and in-person, at the program office, and at the program residences.
Specific Duties
All responsibilities apply equally for residents in Jubilee's transitional houses and studio apartments.
- Provide comprehensive, holistic, wraparound case management services to support the re-entry needs of men and women who have returned to the community following incarceration
- Make linkages and referrals to appropriate agencies, services, and/or organizations to address the client"s sobriety, employment, social services and mental health needs within the program; document progress and maintain contact with all necessary collateral services
- Work in collaboration with the resident and Recovery Services Coordinator, develop individualized Goal Plansto address recovery needs (sobriety, mental health and physical wellness), program goals, employment, housing referrals and budgeting to ensure program success
- Conduct weekly or as needed 1-on-1 and group resident engagements to support resident Goal Plans and residents housed in transitional homes and the seven (7) long-term studio apartments
- Ability to work a flexible work schedule for the purpose of evening meetings is required to accommodate resident work schedules
- Document resident engagements and maintain quality, up-to-date electronic and physical case files
- Assess residents for flex fund opportunities to include benefit distribution and/or processing
- Enter resident engagement activity into two databases – the Efforts to Outcomes (ETO) database utilized by Jubilee's grant funder, Jubilee's Apricot database for in-house service tracking and outcome assessments
- Develop and update a resource/referral list of support organizations that address needs of Program residents; including but not limited to employment, workforce development, housing options, financial resources, entitlements, and healthcare
- Build partnerships with and serve as liaison with alternate community programs, employers, collaborative groups and government agencies to meet the needs of reentering citizens
- Other duties as assigned which are deemed beneficial to the success of program participants
Desired Qualifications
Justice-involved individuals are encouraged to apply. Personal experience returning from incarceration is highly valued by the organization and can be considered in lieu of other qualifications.
- Bachelor's degree in Social Work, Human Services, or related field, although other forms of experience may substitute
- Knowledge of recovery support systems and 12-Step programs required
- Experience working with individuals who are or have been justice involved is preferred.
- Competency in delivering trauma-informed systems of care.
- Interpersonal, engagement, organizational and time-management skills
- Strong written and verbal communication skills
- Ability to support the organization's mission and philosophy, and demonstrate sensitivity to cultural diversity and organizational harmony
- Self-directed and flexible with the ability to meet the needs of the program and residents (must be able to work a flexible schedule to include attending mandatory resident meetings/groups on Tuesday/Wednesday's from 7pm-8pm).
- Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments
- Ability to work a flexible work schedule to meet the needs of working residents
- Ability to travel inside prisons and jail facilities that house men and women
