Reports to: Director of Patient Services
Position Summary: The Regional Clinical Supervisor is a Registered Nurse who is responsible
to perform all clinical duties in their region and/or facilities (and assist in other regions, where
there is a need). Additionally, this role is responsible to oversee all clinical and personal care
staff in their region:
Fulton
Montgomery
Schoharie
Herkimer
Saratoga
Qualifications Requirements: To perform this job successfully an individual must be able to
perform each essential duty satisfactorily. Must be a Licensed Registered Nurse in New York
State, and have at least 1 year of experience as a professional nurse, current BLS certification,
and liability insurance. The requirements listed below are representative of the knowledge, skill,
and /or ability required. In addition, maintaining a professional and positive demeanor at all
times are essential.
Reasoning Ability: Must have ability to solve practical problems and deal with a variety of
concrete variables in situations where only limited standardization exists. Ability to interpret a
variety of instructions furnished in written, oral, diagram or schedule form. Ability to
effectively present information in one-on-one and small group situations to members, their
families and other employees of the organization.
Other Skills and Abilities: Intermediate computer skills required Microsoft word and Excel a
plus. The ability to utilize computer software integral to agency operations after initial training
period and multi-tasking capability is a must
Physical Demands: The physical demands described here are representative of those that must
be met by employee to successfully perform the essential functions of this position. Reasonable

the essential functions. In the home care setting, the employee may be exposed to varying
physical environments and allergens and may be required to lift and/or move up to 25 pounds.
Ability to work in a stressful environment and to effectively deal with stress may be expected.
Duties and Responsibilities:
Conduct visits in the field which may include member homes, adult care facilities and
other locations
Perform assessments which include but are not limited to: Start of Care Assessments, RN
Reassessments, PCW1 Screenings, Companion Screens, UAS Assessment, and
NHTD/TBI Assessments
Supervise and oversee training of caregivers which includes aide supervisions, aide skills
review and plan of care reviews
o Responsible for caregiver clinical re-education when an issue or concern is
identified
Responsible to create all required MD orders and interim orders based on assessments,
reassessments, missed visits and changes in visit frequency
Initiate and/or complete required incident reporting steps when assigned to investigate
and report on an incident relating to a member, case, and/or caregiver
Complete required chart audits for continuous quality improvement
Required to complete all necessary documentation in time frames allotted
o Cleary and concisely document all visits and telephone interactions with
members, families and providers of care including primary care providers and
specialists
Participate in case conferencing, as needed or requested
Attend staff meetings virtually and in-person
Other roles, duties and tasks as assigned by Director of Patient Services or Director of
Operations
