Registered Nurse Assessment Coordinator (Part Time) - Chestnut Hill Lodge Health and Rehabilitation Center

Chestnut Hill Lodge is a skilled nursing facility offering rehabilitation and long term healthcare services. An excellent opportunity is becoming available for an experienced RNAC.

Responsibilities include:

• Coordination of the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA and other payor program requirements.
• Ensures assessments accurately reflect the physical, mental and psychosocial status of each patient and resident; ensures appropriate documentation to report and support services provided and assessment accuracy.
• Communicates effectively with all members of the interdisciplinary team.
• Other duties as assigned.

The ideal candidate will have:

• Experience with MDS completion, reimbursement, clinical resource utilization and/or case management is required.
• One-two years of experience as an RNAC in healthcare is preferred.
• A current Registered Nurse license from Pennsylvania is required; BSN is a plus.
• Experience using PointClickCare or similar healthcare information system is required.
• Proficiency with Microsoft Office applications such as Outlook, Excel and Word.
• Excellent interpersonal and organizational skills
