Supports the Operations Director with the services located in the assigned area(s)
This includes direct patient care, ensures required documentation and worklist are completed by staff in Epic
Ensure compliance with Quality Measures and other applicable regulatory bodies
Works with practice leads in the assigned area(s) to ensure compliance to policies and procedures and clinical protocols
Position will work in conjunction with physicians and leaders for adequate clinical staffing and clinical quality.
Document and review nodules identified on the lung cancer screenings
Appropriate management of lung cancer dashboard
Monitor and report on patient outcomes
Facilitate and triage patient imaging and provider visit appointments to reduce barriers in care
Triage patients to assure timely diagnosis and follow up
Assure work up is complete and communicated
Assist in the Lung nodule clinic as necessary
Complete and record benchmark monitors for quality measure compliance with the Commission on Cancer program
Cover in the absence of the Primary RN Care Coordinator during absences
Monitor and report on patient outcomes
Educate patients and their families about lung conditions, treatment plans, medication adherence, lifestyle changes, and symptom management
Act as a liaison between patients, families, and healthcare providers, advocating for patient needs and ensuring they receive appropriate care
Assist in scheduling patient appointments, procedures, and follow-up visits
Coordinate referrals to specialists or other healthcare services as needed
