Clinical Nurse Supervisor , RN ; Supervises home health care services that are delivered to patients in the comfort of their home in accordance with physician orders and a plan of treatment/care. Services are delivered by Nurses for SOC / Admissions / follow up visits with set goals. The RN continues to implement standards, procedures and processes for delivering care that is safe, therapeutically effective, meets budgeted productivity goals and complies with all regulations. The RN is responsible for adequate clinical utilization to meet current and future demand for services. This individual oversees quality management and performance improvement programs.

Full QA support is provided for daily reviews, of all patient charts to meet CMS / State guidelines.

JOB REQUIREMENTS

Requirements:

* 1 to 2 + years in a clinical leadership role at a Medicare-certified home health care agency.

* Experience managing skilled census of 30+ patients.

* Extensive experience with Medicare/Medicaid home health regulations.

* Experience working with managed care organizations.

* Active and clear RN in Virginia or a compact state.

* Experience coordinating care from multiple disciplines.

* Proven ideas to reduce unnecessary hospitalizations.

* Experience with chronic disease management.

* Flexible Schedule is available.

Must have Experience with CMS/CHAP (Community Health Accreditation Program).

* Must have Experience with Kinnser / Wellsky software.

* Must know CMS guideline, VA HHA guidelines.

Job Type: Full-time

Pay: $85,000.00 - $95,000.00 per year
