Summary:
24 hours per week Outpatient Dialysis Schedule:
Part Time 3 days per week
Shifts consist of: Mon-Wed-Fri 6:30am- 3:00pm Tues-Thurs 6:30am- 5:00pm every other Saturday from 6:30am- 5:00pm and 1 evening shift on a Mon Wed or Fri 2:00pm- 10:30pm
Under the direct supervision of the Clinical Manager (or designee) coordinates the total nursing care for End Stage Renal Disease (ESRD) patients on chronic dialysis by assessing planning implementing and evaluating and in accordance with established hospital and departmental policies and procedures. Provides leadership for the healthcare team by working cooperatively with the other renal team personnel in maintaining a standard of professional nursing practice in the clinical setting. From admission to discharge plan with consideration of the aging process throughout the life span including all developmental stages as well as cultural patterns.
Responsibilities:
Provides direct patient care for those utilizing Outpatient Dialysis services.
Assesses and identifies each patient�s needs for each dialysis session utilizing advanced assessment skills.
Identifies patients needs based on the assessment.
Develops individualized plan of care for a specific dialysis session establishing interventions and patient outcomes in collaboration with physicians other health team members patients and their significant others.
Implements the plan of care which includes initiating and discontinuing dialysis according to proper procedure. Clinical observation and equipment monitoring administration of medications and blood components as ordered.
Documents nursing actions and makes adjustments based on the evaluation.
Troubleshoots dialysis equipment when necessary.
Provides leadership by delegating supervising and participating in safe nursing care activities provided by LPNs - Dialysis and the Patient Care Technicians.
Evaluates effectiveness of patient care and identifies opportunities for improvement.
Assess and provide appropriate teaching based on patient and family needs.
Participates in unit-based quality improvement and research activities.
Utilizes all available resources to provide quality cost effective patient care.
Meets and maintains clinical competencies measured by pre-determined credentialing standards mandatory/safety education and completion of Basic Life Support and ACLS/PALS as appropriate.
Identifies growth needs and participates in educational opportunities and professional organizations.
Serves as a role model/preceptor to new graduate nurses and less experienced personnel.
Assumes charge nurse responsibility for the patient care unit as delegated.
Other information:
Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
BASIC KNOWLEDGE:Baccalaureate in Nursing preferred.
Certified Nephrology Nursing Certification preferred.
Must have current licensure to practice as a Registered Nurse in the State of Rhode Island.
Demonstrates knowledge and skills necessary to provide care to patients throughout the life span with consideration of aging processes human development stages and cultural patterns in each step of the care process.
EXPERIENCE:
Must have a minimum of 12 months of nursing experience and an additional 3 months of specialized dialysis experience providing clinical nursing care to dialysis patients in either a chronic or acute setting.
Experience must include dialysis focus of renal and hematological systems such as managing fluid and electrolyte balances hemodynamics such as anemia management iron management bone disease management and being able to differentiate renal lab values from normal lab values.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Visual and aural ability to monitor patient status.
Physical ability to assist in lifting and positioning patients.
Exposed to patient care environment.
Physical ability for extensive walking and standing; lifting positioning and pushing patients on stretchers.
