Position Summary:
Provides skilled nursing care in accordance with the treatment and plan of care prescribed by patient, family, physician, and members of the interdisciplinary team. The Float Registered Nurse, will provide staffing support to cover admissions, scheduled visits, referral/intakes, crisis care, and/or assist with patient care in other facilities/venues as needed.
Duties & Responsibilities:
- As part of the admissions team, helps to ensure timely admission of referrals to various HiCare programs (hospice, inpatient and contracted facilities).
- Provides backup for HiCare’s various service lines (hospice homecare, inpatient, palliative care).
- Assists in maintaining the 24 hour on-call coverage as needed.
- When admitting a new patient to hospice care, performs the required fields in the Initial Comprehensive Assessment.
- Performs appropriate interventions under M.D. orders, making appropriate contact with M.D. as needed.
- Determines urgency of care as needed. Initiates appropriate interventions in urgent situations.
- Develops the Initial Plan of Care relevant to the immediate patient/family needs.
- Practice clinical standards that comply with HiCare’s mission, vision, values, and policies and procedures.
- Communicates in an effective and timely manner with team members and staff.
- Ensures the safe custody and administration of drugs in accordance with the Standards of Practice.
- Ensures adherence with performance metrics – reports all concerns to the immediate supervisor.
- Actively seeks out and implements evidenced based practices.
- Attends to own personal and professional development needs as identified in personal development plan.
- Maintains a sound working knowledge of the Hospice Conditions of Participation (COPs). Upholds and maintains the organization's cost containment efforts without jeopardizing the quality of hospice care.
- Provides education and training to patients, care givers, volunteers, and other team members as deemed necessary.
- Participates in meetings and committees as requested.
Skill & Knowledge Requirements:
- Ability to exercise independent sound judgment in planning and providing quality nursing care.
- Ability to provide unsupervised health care services in private residences, along with handling and administering controlled and prescribed medications.
- Ability to coordinate care with the interdisciplinary team.
- Requires excellent critical thinking and assessment skills.
- Interest and desire to give care with a special sensitivity to the needs of the dying.
- Maintenance of adequate educational and experiential level to meet the needs of the patients and care givers and regulations established by state and federal laws.
- Positive attitude.
- Possess intermediate knowledge or aptitude to operate a computer, knowledge of basic software programs.
- Strong desire to work as part of a highly interactive team.
- Should be able to prioritize and delegate comfortably.
- Must be a strong patient and family advocate.
- Must be thorough and detail oriented.
- Use ethical principles in decision making and practice.
- Comfortable in maintaining professional boundaries.
Education & Experience Requirements:
- Graduate of a registered nursing curriculum; BSN degree or higher.
- Possession and maintenance of a valid Hawai`i State nursing license.
- Valid CPR certification.
- 2 years’ experience in a healthcare environment or equivalent position.
Hours:
- Regular full-time employee. Block shifts (may start mid-day at times). Schedule determined by Supervisor.
- Duties may require after hour work including weekends and holidays, as needed.
Pay Range: $38.00 - $42.00 per hour.
*This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
