Pediatric Registered Nurse
Under the direct supervision of the Practice Manager and Nurse Manager, the Pediatric Registered Nurse is responsible for providing direct patient care to a diverse pediatric patient population across the continuum of care.
Essential Functions:
- Apply the nursing process by assessing, planning, and carrying out interventions in the management of pediatric patients' care in collaboration with a multidisciplinary care team including medical clinicians, mental health/behavioral health clinicians, case managers, dentists, optometrists, registered dieticians, clinical pharmacists, health educators, etc.
- Document detailed, systematic medical notes of all pediatric patient interactions in the electronic medical record.
- Conduct in-person and telephone triage and advice supported by existing protocols; protocols are approved by the Medical Director and compliant with clinic policy.
- Provide direct nursing care to pediatric patients, ranging from neonate to adolescence.
- Coordinate post-hospital and emergency room follow-up appointments for pediatric patients.
- Review results, including lab and imaging results, for conditions ranging from chronic diseases to sexual health.
- Provide patient and family education and counseling for chronic disease management, medication adherence, preventive care, and wellness promotion.
- Administer medication as ordered by qualified clinicians; routes include oral, inhaled, IM, IV, and intradermal medications and immunizations.
- Complete procedures and patient care as ordered, including venipuncture, wound care, port flushes, etc.
- Respond effectively to medical emergencies and provide appropriate interventions as needed and per organization policy
- Advocate for the needs of pediatric patients and families within the care team and with community partners.
- Schedule patient follow-ups and specialty appointments.
- Coordinate and follow-up with patient calls and EHR messaging within protocol-driven timelines.
- Provide leadership to clinical support staff including MAs and LVNs in a team-based model of care.
- Provide leadership within the Pediatric Care Team Unit and provide pediatric care expertise for other clinical staff.
- Support team members in continuity care for patients at One Community Health.
Additional Duties
- Convey critical assessment data and observations in a professional manner.
- Delegation of tasks as appropriate within applicable scope of practice to LVN or MA. Oversight of delegated tasks to ensure complete, timely and appropriate execution.
- Document all pertinent professional interactions.
- Complete all required documentation comprehensively and in a timely manner.
- Identify conflicts and seek consultation as needed for professional resolution.
- Interact in a collaborative atmosphere with clinicians, clinic staff and laboratory staff.
- Participate in quality improvement activities including annual training in quality improvement.
- This position functions as a member of the One Community Health team and actively collaborates with other clinical and non-clinical staff to ensure care is proactive and coordinated. This collaboration includes participation in daily team huddles, care conferences and training sessions.
Minimum Requirements
- Associate in nursing required
- Current active license to practice as an RN in California.
- Familiarity with pediatric vaccine management
- Active CPR and BLS Certification.
Preferred
- 2 years pediatric clinical experience required; 1-year pediatric acute care experience preferred.
- Bachelor of Nursing preferred
