About the Job
A premier healthcare facility is seeking a Maternal Care Homecare Registered Nurse to join their team. The Registered Nurse will provide healthcare services in patients' homes caring for post-partum mothers, infants and/or pediatric patients who were recently discharged from the hospital and are in need of additional care in the home.
Under the direction of the supervising Assistant Director of Nursing in Community Care, assesses, plans, implements, and evaluates the patient's plan of care. Provides direct skilled nursing care and observes and instructs the patient in their place of residence. May coordinate services of other caregivers.
Responsibilities included but not limited to are:
- Provides individualized skilled Nursing care to the patients
- Provides mother/pediatric and/or caregiver with teaching and involves them in assessment activities.
- Duties include but may not be limited to wound care, including NPWT/VAC, removal of Prevena Vac, Provide Infant, heel-stick for infants with Hyperbilirubinemia, based on MD orders. Assessment for Postpartum Depression, Assessing for signs and symptoms and appropriate follow up for the Pre- eclampsia patient.
- Provides teaching on disease and medication management.
- Provides assessment of patient and develops a plan of treatment and care with ongoing evaluation of patient/family needs for other services.
- Provides professional nursing services as outlined in the patient's plan of treatment and authorized by patient's physician, and provides emergency treatment as needed.
- Utilizes input from patient, caregiver, MD and peers to devise a Nursing Care Plan which is feasible within.
- Evaluates patient's response to plan of care as necessary, consistent with physician's orders.
- Reviews and revises plan of care as necessary, consistent with physician's orders.
- Authorize & coordinate all services including nursing, rehab, respiratory therapy, nutritional services, equipment, professional and non-professional staffing and support services.
- Completes Physician's Orders in a timely manner.
- Completes admission paperwork, including Physician and Nursing Plans of Care (POCs) within 48 hours of visit encounter.
- Reviews clinical records to ensure compliance with departmental and regulatory standard the physical, financial and emotional resources of the patient/caregiver and the Department of Health.
- Obtains written orders for all verbal orders returned.
- Maintain up-to-date patient files with complete records including care plans and re-certifications, reports of all services rendered, progress notes and discharge summaries.
Requirements:
- Active New York State Registered Nurse License
- American Heart Association (AHA) BLS Certification
