Registered Nurse (RN)-Care Coordinator at West Side Health Care District

About the Job
This position would be support for:
Clinic operations including but not limited to Health Plan driven quality programs, HEDIS measurements, other internal and external quality programs, patient education and data management.
The Care Coordinator-RN will work with the Care Coordinator Supervisor to implement patient care plans and assist with provisions of all necessary services for the patient’s medical, behavior and social needs.
If called upon to function in the role of Registered Nurse will perform administrative, clerical, and clinical duties under the direction of a physician and within the positions scope of practice as governed by the Medical Board of California.
Required: ONE of the following must apply
- Possess an active license as a Registered Nurse and worked for at least (2) years in a health care setting OR
- Worked as a Case Manager in a health care setting for at least (2) years.
Other Requirements and Preferences:
- BLS/ CPR Certification (Required)
- Bilingual Spanish/ English-Healthcare Versed (Preferred)
- Case Management (Preferred)
- Active Driver’s License and Proof of state required Auto Liability Insurance (Required)
