Registered Nurse (RN) ? Population Health (Virginia at Home Program) at University of Virginia Health System

Virginia at Home (VaH) offers homebased primary care for older adults who are homebound due to medical, functional, or cognitive issues. The program focuses on patientcentered care, helping participants stay in their preferred environments while reducing unnecessary hospital visits. Services include house calls, telehealth, caregiver support, and Advance Care Planning.
We are looking for a dedicated, compassionate RN to join our team. This fulltime role involves managing the nurse triage line, coordinating scheduled home visits, and addressing daily regulatory requirements from the Virginia at Home office. Other responsibilities include traveling to patients' homes to perform frontline nursing care, when needed, including assessments, lab specimen collection, vaccine administration, and patient education. A reliable vehicle is required for travel within a onehour radius of the main office, with mileage reimbursed.
Capable clinician, focused on expanding knowledge and skills. Consistently provides effective direct care, as part of the interdisciplinary team, to a variety of complex patients. Manages care and implements treatment plans at a refined skill level in collaboration with patients, their families, physicians, and other members of the health care team. Seeks as well as provides feedback for improved clinical practice. Assumes a beginning leadership role but seeks mentoring in this process.

Position Compensation Range: $37.51 $49.82 Hourly

MINIMUM REQUIREMENTS

Education: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.

Experience: 1 year of relevant experience.

Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Healthcare Provider BLS certification required.
