Job Summary:
The Registered Nurse/HCSS Supervisor is responsible for providing professional nursing care to the agency’s home care clients as prescribed by the physician and in compliance with the state’s Registered Nurse Practice Act, any applicable licensure/certification requirement and agency policies and procedures.
Responsibilities:
• Develop and revise care plans and discharge plans in conjunction with clients, families, physicians and care
managers.
• Evaluate care plan for progress towards goals and revise as needed.
• Obtain medical orders to guide plan of care for clients.
• Ensure timely and proper completion of all agency medical related documents and ensure that all documentation
meets NYSDOH regulations and agency standards.
• Complete routine home visits to manage assigned caseload per agency policy.
• Provides supervision of HCSS, PCA, and HHA staff to ensure safety and compliance with plan of care.
• Train and supervise HCSS, PCA, or HHA staff on proper use of durable medical equipment or other care aids.
• Attend team meetings per the request of the Service Coordinator/Care Manager for clients. Communicate and
complete any required reporting to insurance carriers/MLTC care managers for non-waiver clients.
• Use professional nursing skills of assessment, planning, implementation and evaluation when working with
clients/families.
• Demonstrate ability to analyze information received via assessment process.
• Administer and prefill medications for clients.
• Provide medical treatment ordered by the physician for all non-waiver clients.
• Perform agency requested “meet and greets” to ensure appropriateness and ability to provide a safe plan of care in the home environment.
• Recommend and initiate appropriate preventative, therapeutic and rehabilitative procedures and techniques to clients as appropriate.
• Demonstrate awareness of and adherence to waiver, non-waiver, and agency policies and procedures while
ensuring safe and skilled nursing care.
• Perform client’s personal care and/or assist with activities of daily living as needed.
• Provide after hour on-call services to ensure oversight and safety for aide staff and clients.
• Perform other duties as assigned.
Responsible To:
• Clinical Director
Pay: $34 to $36 per hour
Position Requirements:
• Graduate of an accredited School of Nursing.
• Valid and Current New York State Registered Nurse License and Registration.
• Ability to work remotely and also report to work in the office, in a hybrid capacity.
• Previous Home Care experience preferred.
• Ability to successfully complete a Competency & Proficiency Test & Skills Evaluation appropriate to job title before hire.
• Excellent verbal and written communication skills.
• Excellent human relations and customer service skills.
Ability to function with minimal direct supervision, to make decisions, to problem solve, and to manage emergencies.
• Ability to relate to a diverse and multi-cultural populations.
• Appearance is professional and complies with agency dress code.
• Completion of a clear background check.
• Ability to lift up to 25 pounds and to sit, stand, bend, stoop, kneel, pull, climb, reach, and perform repetitive
movements of the upper extremities.
• Pass a physical examination and immunization requirements after being offered employment and complete a health
assessment annually, thereafter, including the first shot of the COVID-19 Vaccination by October 7, 2021, per NYS regulations.
• Possess a current, valid New York State driver’s license and a reliable vehicle.
• Completion of Blossom's orientation program.
