POSITION TITLE: Registered Nurse (RN) Charge Nurse
Job Summary:
The duties and responsibilities of the Registered Nurse include supervising direct care staff, performing direct care according to current standards of practice, state and federal regulations and facility policy while overseeing the health and wellness of assigned residents. Communicating professionally and effectively with all members of the care team, including resident and families.
Job Qualifications:
Education: RN degree from an accredited nursing program, college, or university
Licensure: Licensed R.N. with active license to practice in state of Texas
Experience: Minimum of one year in nursing experience in long term or other health care setting. 2+ preferred.
Skills: Assessment and clinical skills (med surg level), supervisory and communication skills, excellent customer service skills.
Basic computer skills.
Current BLS certification.
Certified Dementia Practitioner Certification preferred - course is offered and required once hired
Transportation: Ability to ensure timely arrival and stable transportation.
Exemption Status: Non-Exempt
Environmental and Working Conditions:
The RN works in a healthcare environment. It is possible that the work may include limited exposure residents and/or patients with AIDS, HIV and hepatitis B viruses involving Risk Classification Categories 1, 2, and 3**. Environmental conditions will fluctuate. To be qualified for the position, applicants and teammates will have the ability to work in the environment conducive to caring for residents, with or without reasonable accommodation and without posing a direct threat to self or others.

- Tasks may involve exposure to blood or body fluids
- Tasks do not involve contact with blood or body fluids but could result in performing a Category 1 task.
- Tasks do not involve any risk of exposure to blood and body fluids.
Physical and Mental Effort:
The RN works in healthcare environments but must also be able to move throughout the buildings and grounds as needed throughout the day. Ability to lift to 40 pounds. Requires working under some stressful conditions to meet care deadlines.
Essential Functions:
Performs admission assessments, interim assessments for residents. |
Assesses/Intervenes/Reports changes in resident's physical/emotional condition, reactions to medications and treatments, attends and participates in care conferences and reports to responsible party or physician as needed. |
Assesses/ monitors assigned residents progress and gives feedback on individual interventions with care team. |
Performs direct resident care requiring nursing skills; including, but not limited to IV, Wound Vacs, Injections, Gastrostomy Feedings, Ostomy care, Tracheostomy Care, surgical drains. |
Carries out/coordinates physician orders in a timely manner. |
Participates and completes discharge assessments and required patient/family teaching. |
Directs/Assigns work for direct caregivers (CNA, CMA) to ensure quality care for residents. |
Communicates clearly and professionally with Family, Resident, Professional Providers and Teammates. |
Requisitions approved materials, supplies, and equipment to carry out planned/ordered care; which may include medication refills being maintain in a timely manner, checking orders for errors and accuracy. |
Maintains a safe environment for residents, including adherence to infection control policies. |
Completes and maintains compliance with Relias Education Courses. |
Performs other assignments and duties as requested. |
