JOB SUMMARY:
The Renal Care Coordinator (RCC) will function as assistant under the supervision of a licensed qualified Social Worker. In this position RCC will assist in performing and coordinating care services for the patients and their families, offering support and information. Focusing on the patient's psychosocial adjustment to end stage renal disease. Additionally, functioning as patient care technician in the dialysis unit when needed.
- Management of patients across the health care continuum
- Proactive approach to patient care (Huddles)
- Support 20 plus different tasks related to Conditions of Coverage and Transitional Care under the guidance of Clinical Nurse Manager and Lead Partner RN
- Allows for systematic and consistent processes across the acute and chronic renal units
- Member of the Interdisciplinary team
- Provide support and guidance with corrective action and plans of correction for internal and external surveys.
- Provide guidance and support with Social Workers to monitor transplant status tracking.
- Provide support and guidance for diabetes education and foot assessments.
- Support fall risk assessments documentation and policy for adherence under the guidance of Clinical Nurse Manger and Lead Partner RN.
- Access Coordinator for renal patients to support reduction of catheters and increase fistula rate.
- Provide support to Clinical Nurse Manger and Lead Partner to ensure Medication reconciliation is monitored monthly.
- Provide support to Clinical Nurse Manager with monitoring immunization.
- Provide support to Clinical Nurse Manager with hospitalization tracking.
- Administer quality patient care in a considerate, respectful manner always.
- Perform all functions conscientiously and in a timely manner within established
REQUIREMENTS:
- Bachelor of Science degree in Healthcare Administration, Social Work, Nursing, or similar field
- Minimum of five years of dialysis experience preferred or strong knowledge of the field of Nephrology.
- Registered Certified Nephrology Nurse or current Dialysis Technician Certification by state and national certification programs.
- Current CPR certification required from the American Heart Association.
- Exceptional time management skills
- Excellent communication skills both verbally and written.
- Critical thinking skills, analytical and works well within a team.
- Must have a good understanding of the Quality Assessment and Performance Improvement (QAPI) process and how to implement Plans of Improvement.
- Be an adult educator and decision maker.
- Must be able to delegate tasks.
- Demonstrated proficiency with Microsoft office applications required.
- Experience with Electronic Medical Records preferred.
