Agape of Appleton, a non-profit human services agency providing residential support services to individuals with differing abilities, is seeking a full-time Residential Case Manager to lead our Level 5 Foster Home supporting two adolescents. The Residential Case Manager provides case management, program management, staff supervision, and direct care support to individuals.
Key Responsibilities for the Residential Case Manager:
- Provide inter-intra agency case management
- Develop, update and implement Individual Support Plans (ISPs) and ensure individuals are receiving appropriate authorized care and services per contractual agreements
- Maximize the social functioning of each individual by promoting community integration
- Provide support and advocacy in an effort to safeguard the human and civil rights of each individual while fostering their dignity and self-worth
- Provide guidance to direct care staff and support to individuals when challenging behaviors occur
- Provide crisis response as needed
- Provide leadership and guidance to direct care staff, including training, coaching, mentoring, and performance management
- Provide direct care support to individuals based on program needs and staffing levels
Qualifications for the Residential Case Manager:
- Ideal candidates will possess a Bachelor’s degree in a human services related discipline, such as social work, sociology, special education, psychology, etc., as well as three to five years of experience in case management, program management, staff supervision, and caregiving. An equivalent combination of education and experience may also be considered.
- Must be at least 21 years of age
- Maintain a valid driver’s license with excellent driving record
- Must pass a thorough background check
- Be mentally and physically capable of assisting supported individuals based on their needs
- Be flexible and adaptable in hours of work, environments, and situations
Competitive Pay and Benefits for the Residential Case Manager:
- $21.00 to $25.00/hour, plus overtime for hours worked in excess of 40 per week
