Job Summary:
Contribute to developing, implementing, monitoring, and updating the individualized rehabilitation plan. Perform therapeutic rehabilitative skill development, and offer services appropriate to meet the consumers' needs.
DUTIES
- Allocate the majority of hours worked toward face-to-face contact with assigned consumers.
- Develop comprehensive service plans that identify specific goals consistent with the consumer's wellness and recovery and the agency's mission.
- Work cooperatively with other social service agencies, health providers, and mental health providers to render quality service through a collaborative treatment approach.
- Transport Consumers.
- Document service contacts, service plans, and service coordination activities on a web-based documentation system.
QUALIFICATIONS
- A Bachelor's Degree in Social Work, Psychology, or other human services-related degree is required
- Valid non-provisional NJ Drivers License with not more than 5 points required
- A minimum of one year of experience working with individuals with a mental illness in a residential setting is required
- Bilingual in Spanish preferred
Must be able to pass a drug test, including non-use of marijuana
Must provide proof of vaccination against COVID-19 or documented religious or medical exemption to apply
