JOB SUMMARY:

The Licensed Practical Nurse (LPN) Shift Nurse performs clinical and non-clinical duties as assigned.  Maintains all medical regulatory and compliance issues for the individuals served by The Arc of SWGA. The Shift LPN maintains compliance with all state and federal policies and procedures.

Non-Exempt Position.

DUTIES AND RESPONSIBILITIES

Screens individuals monthly by using the HRST screening tool (a tool that screens for health risks associated with a wide variety of disabilities).

Monitors behaviors of individuals monthly and daily if needed.

Review individual’s medications and note any changes in the HRST system.

Notes all ER visits and hospitalizations for each individual in the HRST system.

Calculates vital signs and weights monthly and notes information in the HRST tracking sheet for the individual.

Adds doctor visits and new orders to the individual’s HRST profile.

Educates staff on medications and diagnoses for their individuals.

The nurse will be assigned to one individual and will take care of any incidents, medical needs, and PHC paperwork while they are there for the day.

Completes Medical Safety Plans/ Parameters for the individuals and trains the individual and staff on implementing the plan as directed.

Files all information on the individual in the individual’s chart.

Assists in planning and implementing the semi-annual DSP Skills Fair for competency purposes.

Communicate with the case manager any changes with the individual to the HRST.

Attends the pre-ISPs on each individual

Researches and posts important information for staff and individuals (i.e., Eboli, Flu, etc.)

Attends monthly nursing meetings.

Works with Residential DON to complete CAPS, and Critical Incident Reports and to conduct investigations as required by DBHDD as assigned.

Maintains a working knowledge of Community Standards for All Providers, HFR, CLA, PHC, Waiver regulations

Demonstrates expertise in all DBHDD Medically Related Policies and Procedures.

Has a working relationship with pharmacies, physicians’ offices, medical supply companies, and Support Coordination.

Interacts and communicates effectively with all residential teams.

Provides detailed verbal and written reports on assigned functions at all residential, departmental, and managerial meetings.

Provides direct care support as needed.

Completes all staff training as well as other identified training.

Arc Orientation-Policies/Documentation I and Documentation II

Person-Centered Awareness

Medical Private Home Care

Additional RELIAS courses as assigned.

Other duties as assigned.

QUALIFICATIONS:

Required:  3 years of clinical experience and experience with individuals with DD and/or MH.  Must have and maintain LPN Licensure.

He/she must have experience in clerical/office administrative duties.  He/she must know about calculators, personal computers, electronic financial data systems, fax, multiple-line telephone systems, and other business-related machines.  Must be extremely organized, an effective communicator, a team player, able to coach, train, and hold staff accountable, and produce reports promptly.  Must be a self-starter, able to solve problems and provide creative suggestions and solutions.  Must have excellent customer service and verbal and written communication skills.  He/she must have a valid Georgia driver’s license and reliable transportation.

OTHER KNOWLEDGE AND ABILITIES:

He/she must have a working knowledge of The Arc of Southwest Georgia’s Mission and Vision as well as the agency’s programs and services.  He/she must have a knowledge and understanding of the agency’s accreditation process and its value to the organization.  At a minimum, the LPN will attend two related trainings per year in addition to all required annual training.

TYPICAL WORKING CONDITIONS:

The environment will be that of a typical office atmosphere located in the assigned community housing location.  Travel outside the office environment when needed or assigned.

JOB RELATIONSHIPS:

He/She will be supervised by the Residential DON. May receive direction from the Residential RN as needed.

He/she will have no supervisory responsibilities over other employees.
