GENERAL DESCRIPTION:
Utilizes team-based concepts in the day-to-day coordination, supervision, and provision of residential services.
MINIMUM QUALIFICATIONS:
Bachelor's degree in human services or related field and two years supervisory experience; or Associate's degree in human services or related field and three years supervisory experience; or High School Diploma, 5 years human service experience, and three years supervisory experience.
Previous experience in an ID/DD setting is preferred.
Valid/Clean driver's license required.

DUTIES AND RESPONSIBILITIES:
- Supervises and provides training to assigned staff.
- Prepares and conducts performance evaluations as required.
- Ensures adequate staffing of the residence and schedules substitute coverage as needed.
- Conducts regular staff meetings.
- Facilitates development, implementation and monitoring of team-based goals for assigned sites, and the Residential Services program as applicable.
- Provides management of Residential Habilitation Services (including all Behavioral, Health & Safety Plans and Staff Services) for individuals at the residential site.
- Develops, maintains and tracts individual Res Hab Plans. Monitors and reports progress of individual Res Hab services through monthly summary notes. Provides staff training regarding the accurate and consistent implementation/delivery and documentation of Res Hab Services for all participants.
- Participates in Circle of Support meetings and completes required reviews and documentation.
- Develops, monitors, and maintains all individual IPOP's and Site Plan of Protection on a regular basis per agency policy.
- Provides regular staff training on implementation and updated info.
- Attends necessary meetings, training sessions and conferences within the Agency and the community. Acts as liaison with other community agencies.
- Understands and adheres to Agency policies/procedures and ensures all applicable regulatory requirements are met. Ensures adequate staff training regarding the underlying principles of these policies/regulations and their effective utilization in the provision of direct care services.
- Observe and evaluate staff in performance of their job and provide constructive feedback and recognition.
- Responsible for the development of Direct Support staff skill set and professional development.
- Ensures accuracy and completeness of all record keeping in accordance with Agency policy.
- Manages site budget and ensures staff accountability.
- Determines residence needs and ensures timely purchase of goods in accordance with Agency procedure.
PHYSICAL DEMANDS/WORKING CONDITIONS:
- Must be able to tolerate stress associated with uiltiple, simultanious demands.
- Regularly required to talk, hear, stand, sit, walk, bend, stoop, kneel, push, pull and frequently required to use hands and fingers.
- Safely operate a motor vehicle and be able to maintain a valid driver's license.
- Must be able to safely lift and/or move up to 50 pounds.
