Our skilled nursing facility, located in has developed a program that caters to our current and future residents respiratory and cardiac needs by strengthening their bodies that have been debilitated by the heaviness of the pandemic. Policies and processes have been created so we can have a proactive focus on bettering the lives we serve. The ideal candidate for this position will be a Certified or Registered Respiratory Therapist or Registered Nurse, licensed in the State of ,with a love of servant leadership. Alongside the Director of Nursing, Infection Preventionist, and support staff, the Pulmonary Program Coordinator will aggregate and analyze data regarding resident participation, review evaluation of their clinical partners for qualitative and quantitative information, collaborate with the IDT, practioners, residents and family members and execute treatments and interventions as necessary to create an atmosphere of health and wellbeing establishing tangible goals and tracking the progress of those we steward! Principle responsibilities
Under the supervision of the Director of Nursing, the Pulmonary Program Coordinator is charged with working in tandem, as applicable, with the facility Respiratory Therapists, Respiratory Nurses, Infection Preventionist, MDS Coordinator and Director of Rehab to oversee the application of pulmonary program interventions, delegation of duties, evaluations of residents, establishment of goals, and determination of program duration. The position will call for collaboration with MDs and interested parties, care planning, auditing participation, ensuring valid dx are place and all specifics are reconciled for the triple check process, attending any in servicing continuing education or calls involving the program, while work closely with Resource Program Development and Compliance Team to evolve the specifics of approach to achieve the highest resident results.
