Our client, a reputable organization that drives solution to transform the lives of people with disabilities, people in poverty and people suffering from substance abuse/addition, has an immediate need for a Registered Nurse to work on the ACT team
Position Type: HC Contract

SCOPE OF ROLE:
To provide nursing services as a member of a mobile multi-disciplinary treatment team. Organization providing homeless individuals with mental illness with permanent housing and residential support, including psychiatric treatment; individual and group substance abuse treatments; daily living skills training; and financial management skills.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Conducts physical health assessments.
Assesses the health needs of participants and makes the appropriate referrals to community based medical care
Manages and administers intramuscular injections and/or supply medications to consumers as prescribed by psychiatrist.
Provides a range of treatment, rehabilitation, and support services to the participants.
Meets regularly with each participant to assess health status
Provides health management education
Review treatment and medication history
Communicates with the participant's primary medical provider to coordinate medical treatment
When necessary refers them to appropriate services as determined as a result of their assessment.
REQUIRED EDUCATION AND EXPERIENCE:
Licensed Registered RN with in the state of New York.
Must have excellent verbal, written and interpersonal communication skills.
Proficient in MS Word/Excel.
Ability to research material via the internet.
