About the Job
A nonprofit organization that provides comprehensive services and support to individuals with intellectual and developmental disabilities is looking to add a licensed Registered Nurse (RN) to one of its residential facilities in the San Diego area. In this role, the (RN) will be responsible for developing plan of care and coordinating the delivery of healthcare services for persons receiving services in residence.
About the Opportunity:
- Schedule: Monday through Sunday
- Hours: 11am to 7pm (37.5 hrs/wk)
- Setting: Residential Group Home
- Patient Population: Adults
Responsibilities:
The(RN):
- Works under the administrative supervision of the Regional Deputy Director.
- Provides high-quality nursing care to adults and/or children with intellectual and developmental disabilities (I/DD) in assigned programs.
- Assesses, develops, and monitors patient care plans.
- Independently delivers nursing services and comprehensive patient care.
- Collaborates with psychiatrists and community health professionals.
- Coordinates with external healthcare staff to ensure safety, comfort, hygiene, and protection of individuals.
- Treats illnesses and ailments and monitors responses to treatments and medications.
- Provides education to individuals and caregivers and establishes mutual care goals.
- Ensures all actions are consistent with sound nursing practices and judgment.
- Demonstrates and promotes a commitment to the satisfaction, health, wellness, and teamwork of the individuals supported.

Provides nursing services for individuals in assigned programs, collaborating with direct support staff and program leadership to gather updates or information, conducting on-site assessments, and offering training as needed.

Collects data for assessments, including vital signs, social history, primary concerns, and any current complaints of those supported.

Notifies relevant community physicians about complaints from individuals, particularly those that are life-threatening or otherwise significant.

Documents all communications, calls, and encounters with individuals in accordance with guidelines, ensuring entries are made in electronic systems or on paper as required by regulations.

Uses the nursing process to evaluate the health conditions and learning needs of individuals, providing education on treatments, special diets, medications, follow-up diagnostic tests, and healthy living, in coordination with medical providers when necessary.

Triages emergency and non-emergency calls from staff, leadership, or individuals, during and outside of working hours, recommending appropriate actions or destinations (e.g., Urgent Care, ER) and documenting as per protocols.

Observes, records, and reports on patient reactions to medications and treatments, sharing updates with psychiatrists and other medical providers.

Develops and maintains knowledge of Titles 17 and 22 related to the medical care and needs of individuals in the program, along with their specific medical conditions.

Writes, reviews, and updates individualized nursing care plans and restricted health care plans, ensuring all necessary reports and documentation are completed.

Presents information and responds to questions from the IITS Multidisciplinary team and community providers.
Qualifications:
- Graduate of an accredited School of Nursing
- CA Registered Nurse (RN) license
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- * Familiarity with California's Lanterman Act and Title 17 and 22 regulations.
* Two (2) or more years of RN experience working with individuals with I/DD, ideally in a residential setting.
