RN | Cardiac and Pulmonary Rehabilitation - Concord Hospital

Summary

Works with a multidisciplinary team to conduct the Outpatient Cardiac Rehabilitation Program. Performs in a lead role in the provision of telemetry-monitored exercise sessions and coordinates appropriate follow-up with referral sources and the Medical Director as needed. Treatment and education is provided in the areas of deficit to assist patients in safely regaining functional abilities and adopting a heart-healthy lifestyle.

Education

Two years of formal training or education beyond the high school level (e.g., Associates Degree).

Certification, Registration & Licensure

Certification required: Current American Heart Association Basic Life Support (BLS) for Healthcare Providers certification, or equivalent course credentialed by American Heart Association.

Registration required: State of New Hampshire Registered Nurse.

Experience

None required. Must possess knowledge of growth and development across the life span. Ability to effectively and positively interact with a multi-disciplinary staff, patients and others while being a member of a team.

Responsibilities

• Performs Cardiac Rehabilitation Program and Pulmonary GOLD initial evaluations.

• Provides group and individual education related to disease process, medications, risk factors, etc.

• Manages patients in distress.

• Flexible with work assignments.

• Provides clinical monitoring for patients.

• Management of insurance requirements.

• Meets department productivity standard.

• Documents departmental specific requirements.

• Communicates with referral source or Medical Director.

• Fills charge sheets out.

• Participates in conducting Pulmonary Rehabilitation
