Job Description
A Care Coordinator Registered Nurse functions in an ambulatory clinic setting as a member of the clinic’s patient care team that includes clinic registered nurses (RN), licensed practice nurses (LPN), medical assistants (MA), clinic assistants, technicians, clerical staff, physician assistants (PA), nurse practitioners (NP), pharmacists (RPh), and physicians. This position collaborates with inpatient teams, outpatient teams, and the patient and their supports to coordinate a smooth transition from hospital or inpatient stay to home.

Responsibilities:
- Coordinate and facilitate patient centered interdisciplinary care and communication
- Provide regular and consistent follow-up during transitions of care with patient, caregivers, and other healthcare professionals
- Complete medication reconciliation for patients who transition from other healthcare systems
- Ensure records are available for transitional care management visits with primary care
- Work with scheduling to ensure follow-up in clinic within 7-14 days as clinically indicated
- Collaborate with pharmacy to assess resources available for medication affordability
- Maintain relationships with UW Health Skilled Nursing Facility nurse practitioners and case managers to ensure proper transitions following skilled rehab stays
- Collaborate with inpatient discharge planners to anticipate and plan for discharge needs
- Maintain knowledge of community healthcare partners, particularly with home health agencies
- Coach and educate designated patient population and family regarding chronic disease self-management and preventative health maintenance using predefined protocols and evidence based medicine
- Proactively assist providers and other healthcare team members by serving as a patient advocate, ensuring progress toward goal attainment
- Support provider in meeting chronic and preventive health care needs by guiding patients in collaborative self-management
- Partner with social work colleagues to improve outcomes in high-risk patients.
- Utilize critical thinking to make independent judgments related to patient care.
- Maintain responsibility and accountability for the knowledge of conditions of assigned patient populations.
- Follows and complies with all policies, procedures, rules, regulations and Meriter service behaviors.
- Other duties as assigned.
- Graduate of an accredited school of nursing
- Associates Degree in Nursing
- 1-3 years healthcare experience working in an ambulatory clinic setting
- Current license as a Registered Nurse in the state of Wisconsin
- Current CPR certification
- Valid driver’s license when driving any vehicle for work-related reasons.
- Understands the nursing process and can apply to patient care, optimizing patient outcomes
- Understands and supervises delegated tasks to unlicensed personnel
- Enjoys working with variety of patient ages and conditions/diagnosis
- Excellent organizational, internal and external service excellence skills
- Exceptional verbal and written communication skills
- Ability to present oneself as a professional
- Ability to work well in a fast-paced, ever changing environment with multiple priorities
- Ability to work well independently and within teams
- Ability to prioritize work in an efficient manner
- Ability to recognize and respond to urgent needs in a timely manner
- Baccalaureate degree in Nursing
- Knowledge of Epic Electronic Health Record (EHR) System
- Prior knowledge/experience working with regulatory standards (e.g. CMS, Joint Commission)
- Experience in committee membership and participation
- Experience working in a Patient Centered Medical Home care model
- Understands the Patient Centered Medical Home care model
- Understands concepts chronic disease management
